Acceptance and efficiency of various types of back schools have been changing within the recent 15 years. Even though it has been possible to prove, that people who took part in a back school course, were satisfied with the results, it is still questionable in how far there exists a compliance between the costs the courses cause and the improvement of pain from the purely medical point of view. The results are dependent on the design of inquiries and unless they are not standardized it won't be possible to compare the results to each other. The study investigates various studies that carried up to 6123 subjects. Remarkably none of the studies was able to prove that the participants were taught so intense und efficient that they were motivated and able to do their exercises at home and to adopt a back friendly behaviour to their daily life. Medical, psychologial as well as educational aspects are desperately demanded in order to give a complete, satisfiying and long-term succussful treatment to the participants. It is evident that there is an urgent demand of adequate treatment. In order to get this even little children should be tought ways of correct behaviour to prevent back pain or spine deseases. They should be made aware how important daily action is for their personal health and wellness. With adults it is far more difficult: Of course they can develop a certain awarness for a back friendly behaviour but they have to fight against structures of movement that have developed in a very long lasting process. Patience as well as the understanding of neccessity and meaning of daily practicing have to be adopted. The concept of back schools has to ensure individual needs in order to be as efficient as possible for all participants. Even the programme itself should test its own efficiency very critically and correct its methods if neccessary. This is the only way to get a compliant cost-efficieny-relation.
