Chronische und im Anfall akut auftretende Veränderungen des vegetativen Nervensystems sowie in der Erregungsbildung und –leitung des Herzens sind bei Menschen mit Epilepsie gut dokumentiert. Wir untersuchten die periiktalen Veränderungen der Herzaktion bei bilateral tonisch-klonischen Anfällen im Vergleich zu komplex-fokalen Anfällen (KFA). Methoden Am Epilepsiezentrum Hessen in Marburg wurden retrospektiv alle Patienten aus dem Video-EEG-Monitoring (VEM) ausgewertet, bei denen zwischen 2007 und 2015 sowohl bilateral tonisch-klonische als auch komplex-fokale Anfälle aufgezeichnet worden waren. Weitere Selektionskriterien waren die Diagnose einer fokalen Epilepsie und das vollständige Vorliegen einer kontinuierlichen 2-Kanal-EKG-Ableitung. Die Anfälle wurden jeweils 10 Minuten vor Beginn des Anfalles im EEG bis 30 Minuten nach EEG-Anfallsende untersucht. Außerdem wurde die Herzfrequenzvariabilität (HRV) zu zwei interiktalen Zeitpunkten (morgens im Schlaf und im Wachen) bestimmt. Ergebnisse Insgesamt konnten 50 Patienten (mittleres Alter 35,3 Jahre, 50% weiblich) mit einer fokalen Epilepsie (64% temporale Epilepsie, 36% extratemporale Epilepsie) eingeschlossen werden, die die oben genannten Kriterien erfüllten. Bei den bilateral tonisch-klonischen Anfällen zeigte sich iktal eine im Vergleich höhere Herzfrequenz, zudem eine postiktal für 30 Minuten fortbestehende Tachykardie. Nach den bilateral tonisch-klonischen Anfällen zeigte sich eine signifikant verringerte Herzfrequenzvariabilität (HRV). Darüber hinaus stellten wir im interindividuellen Vergleich eine deutliche Verkürzung der QTc-Zeit in den unmittelbar postiktalen Messungen, d.h. eine Minute nach einem bilateral tonisch-klonischen Anfall, fest. Diskussion Eine verringerte Herzfrequenzvariabilität und insbesondere eine verkürzte QTc-Zeit stellen jeweils unabhängige Risikofaktoren für einen plötzlichen Herztod dar. In den bislang berichteten Fällen konnte bei Patienten mit einem SUDEP eine unmittelbar postiktal zentral-vegetativ vermittelte Störung der Atmung und Herzaktion beobachtet werden, in deren Folge es zum plötzlichen Tod kam. Die vorliegenden Daten zeigen eine postiktal vorliegende Störung der parasympathischen Innervation sowie der kardialen Repolarisation und verdeutlichen einmal mehr die entscheidende Bedeutung von bilateral tonisch-klonischen Anfällen für das Auftreten eines SUDEP. Chronic and acute changes in the autonomic nervous system as well as in the formation and conduction of excitation in the heart are well documented in people with epilepsy. We investigated periictal changes in heart action in bilateral tonic-clonic seizures compared to complex-focal seizures. Methods At the epilepsy centre in Marburg all patients from the epilepsy monitoring unit who had bilateral tonic-clonic as well as complex-focal seizures between 2007 and 2015 were retrospectively evaluated. Further selection criteria were the diagnosis of focal epilepsy and the complete presence of continuous 2-channel ECG. The seizures were examined in the EEG 10 minutes before the onset of the seizure until 30 minutes after the end of EEG seizure. In addition, heart rate variability (HRV) was determined at two interictal times (in the morning during sleep and during wakefulness). Results In total, 50 patients (mean age 35.3 years, 50% female) with focal epilepsy (64% temporal epilepsy, 36% extratemporal epilepsy) could be included who met the above criteria. In bilateral tonic-clonic seizures, the ictal heart rate was comparatively higher and the tachycardia persisted for 30 minutes. After the bilateral tonic-clonic seizures a significantly reduced heart rate variability (HRV) was observed. In addition, we found a significant reduction in QTc time in the immediate postictal measurements, i.e. one minute after a bilateral tonic-clonic seizure. Discussion Reduced heart rate variability and, in particular, a shortened QTc time each represent independent risk factors for sudden cardiac death. Previous reported cases of patients with a SUDEP were observed to have an immediate postictal central-vegetative mediated cardiorespiratory dysfunction, resulting in sudden death.
