Ossowski, Ramona Ossowski Ramona Kardiovaskuläre Erkrankungen, insbesondere die Koronare Herzkrankheit, führen die Todesursachenstatistiken in Industrienationen an [64]. Viele Risikofaktoren der Erkrankung, wie zum Beispiel ein gestörter Glukosemetabolismus sind beeinflussbar. Um die Sekundärprävention zu verbessern, gleichzeitig aber auch eine wirtschaftliche Behandlung zu gewährleisten wurde 2003 das Disease-Management-Programm Koronare Herzkrankheit initiiert. In der vorliegenden Arbeit wurde untersucht, ob ein signifikanter Unterschied in der Nachbehandlung zwischen Patienten, die innerhalb des Disease-Management-Programms Koronare Herzkrankheit behandelt werden, und denen, die nicht am Disease-Management-Programm teilnehmen besteht. Desweiteren wurde ein Immunoassay zur Bestimmung des HbA1c auf seine Eignung als mögliches Screeninginstrument, zur Erkennung von Patienten mit gestörtem Glukosemetabolismus, untersucht. Das untersuchte Patientenkollektiv setzte sich aus 38 Patienten zusammen. Einschlusskriterien der Untersuchungen waren eine stabile KHK und eine PTCA oder AC(V)B vor mindestens einem Jahr. Ausgeschlossen wurden Patienten mit diagnostiziertem Diabetes mellitus und Patienten mit instabiler KHK. Die Daten wurden während einer Routineuntersuchung erhoben und durch Akteneinsicht für die vorliegende Arbeit erfasst. 11 Patienten wurden im Rahmen des Disease-Management-Programms behandelt, 27 nahmen nicht am Behandlungsprogramm teil. Die Ergebnisse zeigten, dass die Unterschiede zwischen beiden Behandlungsgruppen gering sind: In Bezug auf Glukosemetabolismus, BMI und Blutdruck bildeten sich keine Vorteile der DMP Gruppe ab. Lediglich die gemessenen Lipidwerte und die medikamentöse Therapie scheinen in der DMP Gruppe etwas besser zu sein. Die Ergebnisse wurden mit einer Arbeit von Bestehorn et al. verglichen. Es zeigte sich, dass in der bundesweiten Studie mit über 45000 Patienten die Unterschiede zwischen beiden Gruppen ausgeprägter waren. Eine mögliche Begründung liegt in den unterschiedlichen Ausgangsbedingungen beider Arbeiten: Das in der vorliegenden Arbeit untersuchte Patientenkollektiv bestand aus Patienten einer sehr ländlichen Region, die in der Arbeit von Bestehorn et al. untersuchten Patienten stammen hauptsächlich aus urbanen Gegenden. Man kann vermuten, dass auf dem Land die Bindung an den Hausarzt enger ist als in der Stadt. Als Folge kann ein Hausarzt, der seinen Patienten schon über Jahrzehnte kennt auch ohne ein strukturiertes Behandlungsprogramm eine auf den Patienten abgestimmte Sekundärprävention gewährleisten. Auffällig war der große Anteil übergewichtiger Patienten und die hohe Zahl von 8 neu diagnostizierten Diabetikern. Beide Punkte bieten Potenzial die Sekundärprävention zu verbessern. Ein mögliches Instrument hierfür könnte das Immunoassay DCA Vantage von Siemens® sein. In der vorliegenden Arbeit wurde untersucht, ob das Gerät zur Bestimmung des HbA1c ein praktikables Verfahren darstellt um Diabetiker frühzeitig zu erkennen und behandeln zu können. Das Messverfahren ist sehr genau und kaum aufwendiger als eine Blutzuckerbestimmung. Dies könnte unter anderem dazu beitragen die Sekundärprävention bei KHK-Patienten zu verbessern. For this reason it is important to treat modifiable risk factors as best as possible to improve secondary prevention. In 2003 the German Disease Management Program for coronary heart disease was introduced. Within the scope of this thesis it is analyzed in how far patients in the disease management program are treated in a better way than patients who do not take part in that program. Data of 38 patients were analyzed. 11 of these patients took part in the disease management program, 27 patients did not. It can be shown that in most parts there was no difference between both groups. Only blood lipids and some parts of medication were treated slightly more consequent within the DMP. The results are compared to the results of another study which was conducted by Bestehorn et al. [5] in 2009. About 45,000 patients were examined in order to compare patients participating in a DMP and patients who were treated outside the DMP. The results are similar to the results in the thesis at hand, however in the study of Bestehorn et al. the benefits for the patients who took part in the DMP were greater. Another aim of this thesis was the analysis of an immunoassay that allows easier measurement of the HbA1c in comparison to standardized measurement methods. The results show, that the new method is as exact as the standardized measurement.
