Aim: The aim of the study is to evaluate the implant survival rate, success rate and the prevalence of periimplant mucositis, peri-implantitis in dependence of varying risk factors in patients suffering from generalized aggressive periodontitis (GAgP) who were treated with telescopic double crowns retained on implants. Analyzed risk factors were gender, age, implant topography, material of the telescope, type of denture, quality and quantity of the bone, augmentation and implant length.
Methods: The case series examines 16 patients treated for GAgP with 83 dental implants within a double crown retained denture over 6 to 21 years. The patients were integrated in a 3-month recall schedule. Examined was radiologic determined bone loss after 3,5 and 10 years as well as the clinical parameters plaque index, gingival index, probing depth and attachment loss. This study determines the implant success rate, defined as the number of implants still in situ. Implant success was defined as no implant mobility, patient being pain free and without discomfort, foreign body sensation or paresthesia, probing depth ≤ 5mm without bleeding on probing, no periimplant radiographic translucence or more than 0,2mm annual vertical bone resorption after the first year in function. Implants that did not meet one or more of these criteria would be score das failure.
Results: The determined plaque index was averaged over the entire observation period at 0,5 and the gingival index at 0,9. The average attachment loss was 4,3mm on implants, the probing depth 3,6mm. The radiological examined bone loss was 1,37mm (8,6%) after 10 years. Implant survival rate was 100%. 69% of the implants have been rated as successful at its last examination. A periimplant mucositis has been diagnosed at 65% of the implants, a periimplantitis at 27.7%. Risk factors for implant success were female gender and implantation in the anterior region of the maxilla. Risk factors for periimplant mucositis were age, implantation in maxilla region and the combination of teeth and implants within one denture. For periimplantitis was only female gender identified as a risk factor.
Conclusion: These results show that the double crown concept on implants or implants and teeth combined can be a sufficient restoration for patients with a treated generalized aggressive periodontitis. Long term success can be expected if a regular recall is observed.
