Aim of the Study The purpose of this study was to establish an evidence-based database in order to get an overview on the longevity of posterior composite restorations. At the same time, a comparison of the results was made with the clinical performance at the Department of Operative Dentistry at the Philipps University of Marburg. Furthermore this study is ought to be a basis for the guideline of the German Society of Dental, Oral and Craniomandibular Sciences (DGZMK) concerning the evidence on posterior composites. Methods A PubMed search as well as an additional manual search for articles were performed. Studies published between 1955 and 2013 were considered. The search resulted in 34 selected studies. Results The estimated median survival rate was 9.2 years which corresponds with the results of the Department of Operative Dentistry at the Philipps University of Marburg (8.9 +- 1.8 Years). It could be clearly shown that the longevity of restorations depends on many factors. The length as well as the design of a study have a high influence on the results. Factors related to the operator, patient or cavity are of great importance. This study also confirms that the hybrid composites as well as the three-step adhesive systems should still be considered the “ gold standard“. Significance/Conclusion In conlcusion, composite restorations have shown a favorable performance in posterior teeth. Success and quality of these restorations depend on a number of factors, which need to be stongly respected in order to enable a high longevity.
