Zusammenfassung Obwohl es in der Behandlung des Mammakarzinoms deutliche Fortschritte gibt, bleiben die Therapieoptionen für Hormonrezeptor-negative und ossär metastasierte Tumoren begrenzt. Die Entdeckung des RANKL/RANK/OPG-Systems im Jahre 1997 hat das Verständnis über die komplexe Regulation des Knochenstoffwechsels grundlegend beeinflusst. Das Zytokin RANKL, sein Rezeptor RANK und der Rezeptorantagonist OPG sind essenzielle Regulatoren der Osteoklastenbiologie. Zunehmend wird auch die Bedeutung von OPG und RANKL in der Tumorbiologie akzeptiert. So konnte gezeigt werden, dass RANKL die Migration von Brust- und Prostatakarzinomzellen in vitro und in vivo fördert. TRAIL ist ein Mitglied der TNF-Familie, das in malignen Zellen Apoptose induziert. Als Rezeptorantagonist für sowohl RANKL als auch TRAIL, besitzt OPG prinzipiell die Fähigkeit, zwei fundamentale Prozesse der Tumorbiologie zu beeinflussen, die Apoptose und die ossäre Migration. Östrogene regulieren vielfältige Zytokine und Wachstumsfaktoren in Östrogen-Rezeptor (ER)-positiven Brustkrebszellen. Im ersten Teil dieser Arbeit wurde die Regulation des RANKL/OPG-Systems durch Östrogene und Androgene in der ER-positiven MCF-7- und der ER-negativen MDA-MB-231-Brustkrebszelllinie untersucht. In den hauptsächlich ER-α-exprimierenden MCF-7-Zellen führte die Exposition mit 17β-Estradiol und Testosteron zu einer dosisabhängigen Hemmung der OPG-mRNA-Expression und -Proteinsekretion um 70 bzw. 65%. Ferner konnte die Expression von RANKL in beiden Zelllinien nachgewiesen werden. Während IL-1β in beiden Zelllinien zu einer Induktion von RANKL führte, unterdrückte 17β-Estradiol die RANKL-Spiegel der MCF-7-Zellen, nicht aber der MDA-MB-231-Zellen. Die Suppression der OPG-Produktion durch 17β-Estradiol und Testosteron konnte durch die Gabe des Anti-Östrogens ICI 182,780 verhindert werden. Zudem konnte der Testosteron-Effekt durch einen Aromataseinhibitor blockiert werden, was darauf hinweist, dass Androgene die OPG-Spiegel in Brustkrebszellen indirekt nach einer Aromatisierung und Bindung an den ER beeinflussen können. Im zweiten Teil wurde die Expression und Modulation von OPG und TRAIL durch IL-1β und Dexamethason in den MCF-7- und den MDA-MB-231-Zellen untersucht. In beiden Zelllinien, wurden die OPG-mRNA-Spiegel und -Proteinsekretion zeit- und dosisabhänig durch IL-1β gesteigert und durch Dexamethason gehemmt. Im Gegensatz zu den MCF-7-Zellen exprimierten die MDA-MB-231-Zellen TRAIL, welches durch IL-1β gesteigert und durch Dexamethason unterdrückt wurde. TRAIL aktivierte die pro-apoptotischen Caspase-3,-7 und PARP und führte zum Zellverlust in den MDA-MB-231-Zellen, hatte jedoch keinen Einfluss auf die MCF-7-Zellen. Der OPG-Knock down mittels siRNA führte zu einer um 31% gesteigerten Apoptoserate im Vergleich zu mit Kontroll-siRNA behandelten MDA-MB-231-Zellen. Zudem schützte das ins konditionierte Medium sezernierte OPG die Zellen vor der TRAIL-vermittelten Apoptose. Dieser protektiven Effekte konnten durch die Blockade von OPG mit seinem spezifischem Liganden RANKL aufgehoben werden. Die Assoziation zwischen Zellschutz und OPG-Produktion in den MDA-MB-231-Zellen wurde ferner durch die Beobachtung unterstützt, dass die Modulation der OPG-Sekretion mittels IL-1β und Dexamethason vor einer TRAIL-Exposition zu einer Senkung bzw. Steigerung der Apoptose führte. Bisphosphonate sind etablierte Medikamente zur Behandlung von Knochenmetastasen, welche die Osteoklastenaktivität hemmen und so den Circulus vitiosus aus Osteoklast-Tumorzell-Interaktion unterbrechen. Im dritten Teil dieser Arbeit wurden die direkten Effekte von Zoledronsäure auf die MDA-MB-231- und MCF-7-Zellen evaluiert. Während Zoledronsäure (100 µM) die Zellproliferation der MDA-MB-231-Zellen nach 72 h hemmte und die Apoptose über eine Aktivierung von Caspase-3 und -7 induzierte, zeigten sich nur geringe Effekte in den MCF-7-Zellen. Des Weiteren steigerte Zoledronsäure die Expression von TRAIL in den MDA-MB-231-Zellen, hatte jedoch keinen Einfluss auf die Expression seines Rezeptorantagonisten OPG. In den MCF-7-Zellen wurden beide Zytokine durch die Gegenwart von Zoledronsäure gehemmt. Zusammenfassend wurden in dieser Arbeit zwei potentielle Ergänzungen zur Therapie des Mammakarzinoms erforscht. Zum einen wurde der direkte pro-apoptotische Effekt von Zoledronsäure auf ER-negative Brustkrebszellen nachgewiesen und weiter geklärt. Zum anderen wurde die vielfältige Regulation des OPG/TRAIL/RANKL-Systems durch karzinomrelevante Substanzen untersucht. Die Bedeutung der endogenen OPG-Sekretion als Überlebensfaktor für TRAIL-sensitive Mammakarzinomzellen wurde dargestellt sowie die Möglichkeit diese Fähigkeit therapeutisch durch eine gezielte pharmakologische Hemmung der OPG-Sekretion zu nutzen. The discovery of the RANKL/RANK/OPG system in 1997 has fundamentally influenced our knowledge about the complex regulation of bone metabolism. The cytokine RANKL, its receptor RANK and the decoy receptor OPG are essential regulators of osteoclast biology. OPG is also thought to play a role in tumor biology. It was shown that RANKL enhances the migration of breast and prostate cancer cells in vitro and in vivo. TRAIL is a member of the TNF ligand family that selectively induces apoptosis in malignant cells. As a decoy receptor for both RANKL and TRAIL, OPG has the unique ability to regulate two fundamental aspects of tumor biology, apoptosis and migration. Estrogen regulates various cytokines and growth factors in estrogen receptor (ER)-positive human breast cancer. In the first part of this study, the regulation of the RANKL/OPG system by estrogens and androgens in the ER-positive breast cancer cell line MCF-7 and the ER-negative breast cancer cell line MDA-MB-231 were analyzed. In MCF-7 cells, which predominantly express ER-, 17β-estradiol and testosterone dose-dependently decreased OPG mRNA levels and protein secretion by 70 and 65%, respectively. Furthermore, both cell lines expressed RANKL. While IL-1β induced RANKL expression in both cell lines, 17β-estradiol decreased RANKL expression only in MCF-7 cells. The inhibition of OPG production by 17β-estradiol and testosterone was specifically prevented by the pure anti-estrogen ICI 182,780, and the testosterone effect was prevented by an aromatase inhibitor. This indicates that androgens indirectly influence OPG expression in breast cancer cells after aromatization and binding of the ER. In the second part of this study the expression and modulation of OPG and TRAIL by IL-1β and dexamethasone were analyzed in MCF-7 and MDA-MB-231 cells. In both cells, OPG mRNA levels and protein secretion were dose- and time-dependently enhanced by IL-1β and suppressed by dexamethasone. In contrast to MCF-7 cells, MDA-MB-231 abundantly expressed TRAIL mRNA, which was enhanced by IL-1β and inhibited by dexamethasone. TRAIL activated pro-apoptotic caspase-3, -7, and poly-ADP-ribose polymerase and decreased cell numbers of MDA-MB-231, but had no effect on MCF-7 cells. Gene silencing using siRNA directed against OPG resulted in a 31% higher apoptotic rate compared to non-target siRNA-treated MDA-MB-231 cells. Furthermore, TRAIL induced significantly less apoptosis in cells cultured in conditioned media (containing OPG) compared to cells exposed to TRAIL in fresh medium lacking OPG and these protective effects were reversed by blocking OPG with its specific ligand RANKL. The association between cancer cell survival and OPG production by MDA-MB-231 cells was further supported by the finding that modulation of OPG secretion using IL-1β or dexamethasone prior to TRAIL exposure resulted in decreased and increased rate of apoptosis, respectively. Bisphosphonates are established drugs in the treatment of bone metastasis that inhibit osteoclast activity and interrupt the vicious cycle of osteoclast-tumor cell interactions. In the third part of this study the direct effects of zoledronic acid on MDA-MB-231 and MCF-7 breast cancer cells were evaluated. While zoledronic acid (100 µM) inhibited MDA-MB-231 cell proliferation after 72 h, and induced apoptosis via activation of caspase-3 and -7, it had only minor effects in MCF-7 cells. In addition, zoledronic acid induced the expression of TRAIL in MDA-MB-231 cells, but had no effect on the expression of its decoy receptor OPG. In MCF-7 cells, both cytokines were suppressed by zoledronic acid. In summary, this study investigated two potential concepts to target breast cancer. First, the direct apoptotic effects of zoledronic acid and its underlying mechanisms on the ER negative MDA-MB-231 cells were demonstrated. Second, the regulation of the OPG/TRAIL/RANKL system by multiple disease relevant substances was analyzed.
