Zusammenfassung:
Hintergrund und Ziele der Studie Intrinsisch verfärbte Zähne können mithilfe eines intrakanalär eingebrachten Bleichmittels aufgehellt werden. Voraussetzung für eine langzeiterfolgreiche Behandlung ist eine adäquate Wurzelkanalfüllung und anschließender koronaler Verschluss der Eingangskavität mit Komposit. Entscheidend für den Langzeiterfolg ist die Haftung des Befestigungskomposits am Dentin. In dieser In-vitro-Studie soll geklärt werden, inwiefern der Haftverbund nach Einlage der internen Bleaching-Materialien Natriumperborat und Perfect Bleach Office+ im Wurzelkanal die Verwendung adhäsiver Befestigungskomposite beeinflusst. Hierzu wurden die Komposite BiFix SE und Rebilda DC verwendet. Materialien und Methoden Grundlage dieser Versuchsreihe waren 70 humane, einkanalige, füllungsfreie Zähne mit geradem Wurzelverlauf und möglichst rundem Wurzelquerschnitt. Nach erfolgter Dekoronation wurde der Wurzelkanal maschinell aufbereitet. Die Vorbereitung der Versuchszähne war abgeschlossen, sobald eine identische Arbeitslänge von 8 mm vorlag. Von den 70 Versuchszähnen wurden 48 den beiden zu testenden Bleaching-Materialien zugeordnet, wobei 24 Zähnen mit Natriumperborat und 24 mit Perfect Bleach Office+ (VOCO) präpariert wurden. 22 Zähne ohne Bleicheinlage dienten als Kontrollgruppe. Nach dem 7-tägigen Bleichvorgang wurden alle Gruppen mit einem identischen Spülprotokoll gereinigt. Jeder Versuchszahn sollte mit einem angepassten und konditionierten Stahlspreader in den Wurzelkanal verklebt werden. Dazu wurden die drei Versuchsgruppen erneut hälftig aufgeteilt. Der einen Hälfte wurde das Befestigungskomposit BiFix SE (VOCO), der anderen Rebilda DC (VOCO) zugeteilt. In der Zwick-Universalprüfmaschine wurde nach einer 7-tägigen Aushärtezeit der Zugversuch durchgeführt. Dabei wurde der aufgebrachte Kraftaufwand, der benötigt wird, um den Spreader aus dem Wurzelkanal zu extrudieren, dokumentiert. Ergebnisse Die höchsten Haftwerte erzielten Perfect Bleach Office+ und Rebilda DC als Befestigungskomposit (8,17 MPa). Vergleichbar dazu schnitten auch die Gruppen Natriumperborat mit Rebilda DC (6,72 MPa) und die Kontrollgruppe mit BiFix SE (7,82 MPa), sowie die Kontrollgruppe ohne Bleicheinlage und Rebilda DC (7,63 MPa) ab. Lediglich der Einsatz der Bleichmittel Natriumperborat und Perfect Bleach Office+ in Verbindung mit BiFix SE wiesen signifikant geringere Haftwerte im Vergleich zu den anderen Gruppen auf. Schlussfolgerung Aufgrund der Ergebnisse dieser Studie kann davon ausgegangen werden, dass nicht die Wahl des Bleichmaterials (Natriumperborat oder Perfect Bleach Office+) über den Verbund zwischen Wurzelkanaldentin und Spreader entscheidet, sondern die Kombination mit dem verwendeten Befestigungskomposit. Die Haftwerte von Rebilda DC und BiFix SE ohne Verwendung einer Bleicheinlage waren miteinander vergleichbar. Soll jedoch ein interner Bleichvorgang durchgeführt werden, wird die Verwendung von Rebilda DC empfohlen, da die Haftwerte durch den Bleichvorgang, sowohl mit Natriumperborat als auch Perfect Bleach Office+, kaum verändert wurden.
