Zusammenfassung:
Die Gewinnung von Atemwegskondensat (Exhaled Breath Condensate, EBC) ist eine viel versprechende Methode, um nichtinvasiv Zugang zum epithelialen Flüssigkeitsfilm der Atemwege zu erlangen. Mangels hinreichender Standardisierung hinsichtlich methodischer Aspekte bleibt die EBC Gewinnung bislang allerdings wissenschaftlichen Fragestellungen vorbehalten ist und wird nicht in der klinischen Routinediagnostik eingesetzt. In der vorliegenden Arbeit wurde bei gesunden Probanden erstmals die Vergleichbarkeit der gängigen kommerziell erhältlichen Geräte RTube™ und ECoScreen Turbo anhand der EBC Zielparameter pH, Leitfähigkeit und Gesamtproteinkonzentration überprüft. Weiterhin wurde in dieser Arbeit der Einfluss unterschiedlicher Atemmanöver (Ruheatmung und forcierte Atmung) auf die Proteinmessung untersucht. Zusätzlich zur Gesamtproteinkonzentration wurden hierfür als spezifische Proteine das Surfactant Protein-A, das Club Cell Protein sowie das Alpha-1-Antitrypsin im EBC gemessen. Die Studie umfasste fünf Versuchsreihen, in denen jeweils von 10-12 gesunden Probanden EBC gesammelt wurde. Die Probanden atmeten im „Cross-over“ Design jeweils zehn Minuten über den RTube™ und ECoScreen Turbo. Pro Gerät wurde diese Messung in Ruheatmung und Hyperventilation durchgeführt. Es konnte gezeigt werden, dass die beiden kommerziell erwerbbaren Geräte RTube™ und ECoScreen Turbo für die Analyse von pH und Leitfähigkeit im EBC gleichwertig angewendet werden können. Bezüglich der gemessenen Ge- samtproteinkonzentrationen fand sich beim Gerätevergleich ebenfalls kein statistisch signifikanter Unterschied. Unterschiedliche Atemmanöver beeinflussen allerdings die Gesamtproteinkonzentration im EBC. So konnte gezeigt werden, dass forcierte Atmung zu höheren Gesamtproteinkonzentrationen im EBC führt. Mit der Zielsetzung, mehr über den Ursprungsort des EBCs zu erfahren, wurden in der vorliegenden Arbeit die Konzentrationen eines alveolären (Surfactant Protein A, SP-A) und bronchialen Markerproteins (Club Cell Protein, CCP) nach ruhiger und forcierter Atmung im EBC bestimmt. Da es nicht gelungen ist, sowohl das Club Cell Protein als auch das Surfactant Protein A reproduzierbar im EBC nachzuweisen, kann die Frage nach dem Ursprungsort des EBC aktuell nicht beantwortet werden. Ergänzend wurde in der vorliegenden Arbeit Alpha-1-Antitrypsin im EBC gemessen. Die Messungen wurden mit beiden Gerätschaften durchgeführt. Für den ECoScreen Turbo gibt es bislang keine Vergleichsdaten. Es konnte gezeigt werden, dass sich die mit beiden Geräten gemessenen AAT-Konzentrationen bei Ruheatmung im EBC nicht statistisch signifikant unterscheiden. Betrachtet man die unterschiedlichen Atemmanöver, so fanden sich nach forcierter Atmung höhere AAT Konzentrationen im EBC als nach ruhiger Atmung. Bislang gibt es keine Vergleichsarbeiten, die die AAT-Konzentration im EBC nach Hyperventilation untersuchte. Eine letzte wichtige Beobachtung dieser Arbeit betrifft die Standardisierung der Proteinanalyse im EBC. Es konnte gezeigt werden, dass sich die Ergebnisse der Proteinmessung bei einer Normalisierung bezüglich des ventilierten Volumens teilweise verändern. Für zukünftige Forschungsprojekte ist deshalb eine Standardisierung des ventilierten Volumens dringend zu empfehlen.
