Zusammenfassung:
Die Pars petrosa des Os temporale stellt aufgrund ihrer komplexen Anatomie hohe Anforderungen an die radiologische Diagnostik von Ohrerkrankungen. Bis heute gilt hier die konventionelle Computertomographie (MDCT, engl.: multi-detector-row-CT) als Goldstandard. Nachdem vor mehreren Jahren die Methode der Digitalen Volumentomographie (DVT, engl.: cone beam computed tomography = CBCT) Einzug in die HNO-Heilkunde hielt, ist sie zunächst in der Frontobasis vielfach zur Anwendung gekommen. Dagegen wird im Bereich der Laterobasis die Schnittbildgebung derzeit weiter kontrovers diskutiert. Studien für dieses Anwendungsgebiet sind bislang hauptsächlich in vitro durchgeführt worden, während Erhebungen an Patienten erst vereinzelt vorliegen, sodass genauere Angaben über die Vorteile bzw. Limitationen der DVT in vivo noch nicht eindeutig belegt sind. Ziel dieser Studie war es primär, herauszufinden, wie gut einzelne Mittel- und Innenohrstrukturen, wie etwa die filigrane Ossikelkette, mit der DVT am Patienten zu beurteilen sind. Hierzu wurden 228 DVT-Datensätze von Patienten, die eine Bildgebung der Laterobasis erhielten, retrospektiv hinsichtlich der Darstellung von 15 klinisch relevanten anatomischen Strukturen anhand einer drei- bzw. vierstufigen Skala ausgewertet, bei Vorhandensein einer knöchernen Begrenzung der Struktur ebenso diese. Zudem wurden anatomische Varianten erhoben und intra- und intervariable Korrelationsanalysen durchgeführt. Die Ergebnisse waren wie folgt: Der Summenscore als rechnerisches Hilfsmittel zum Grad der Beurteilbarkeit aller ausgewählten Parameter lag im Durchschnitt mit 21,9 ± 4,1 Punkten im oberen Viertel der Bewertungsskala (schlechteste Beurteilbarkeit: 43 Punkte; beste Beurteilbarkeit: 15 Punkte). Die größeren in dieser Arbeit untersuchten anatomischen Parameter konnten in über 95% der Fälle gut mit der DVT dargestellt werden (N. facialis, langer Ambossfortsatz, Fensternischen, Bogengänge und Bulbus venae jugularis superior). Die feinen Komponenten der Ossikelkette waren hingegen weniger deutlich zu erkennen (Gelenkspalten ̴ 50%, Stapesfeinstrukturen zu 17-28% komplett [28-41% teilweise] beurteilbar). Es zeigte sich, dass die Beurteilbarkeit der evaluierten Strukturen nicht altersabhängig war. Bei der Erhebung anatomischer Varianten kamen Dehiszenzen im Bereich des N. facialis häufig vor (50%), wesentlich seltener bei den Bogengängen (5%) und dem Bulbus venae jugularis (7%). Sieben Prozent der Bulbi standen durchschnittlich 2,4 ± 0,7 mm über dem äußeren Gehörgangsboden. Der Durchmesser des inneren Gehörgangs an seiner Ampulle betrug im Mittel 5,1 ± 0,8 mm. Während das Alter nicht mit der Gehörgangsweite korrelierte, bestand bei den Geschlechtern ein signifikanter Unterschied: die durchschnittliche Differenz zwischen weiblichen und männlichen inneren Gehörgängen betrug 0,4 mm (m>w). Die Beurteilbarkeit der Aufnahmen von lebenden Patienten ist im Vergleich zu Aufnahmen von Felsenbeinpräparaten aufgrund diverser Röntgenphänomene und Bewegungsartefakte vermindert. Die Ergebnisse legen dar, dass die DVT zur Beurteilung knöcherner Mittel- und Innenohrstrukturen prinzipiell gut geeignet ist, zeigen jedoch auch die Limitationen dieser Methode auf, besonders in der Darstellung filigraner Mittelohrkomponenten wie z.B. der Stapesstrukturen. Dieser Umstand wurde auch in anderen in-vivo-Studien beobachtet. Da je nach Anwendungsprotokoll die DVT gegenüber der MDCT bei mindestens vergleichbarer bis teilweise überlegener Darstellungsqualität eine geringere Strahlenexposition für den Patienten bedeutet und zudem (wie in anderen Studien gezeigt) weniger Metallartefakte bei der Visualisierung von Mittel- und Innenohrimplantaten auftreten als bei der MDCT, sollte der DVT in Zukunft eine bedeutendere Rolle in der Felsenbeinbildgebung zukommen. Weitere in-vivo-Untersuchungen mit größerem Patientenkollektiv, mehreren erfahrenen Untersuchern und einheitlichen Geräteparametern sind erforderlich, um Standards zum Einsatz der DVT in der Laterobasis festzulegen.
