Zusammenfassung:
Hintergrund und Ziele Eine Wurzelkanalbehandlung ist eine Routineversorgung in der zahnärztlichen Praxis. Sie ist bei einer apikalen Entzündung oder sehr tiefen Karies meist die einzige Möglichkeit, einen Zahn langfristig zu erhalten. Die Erfolgsquote für die endodontische Behandlung ist in den letzten Jahren deutlich gestiegen. Heute werden 10-Jahres-Überlebensraten von 85-93% angegeben. Der Langzeiterfolg und damit die Zufriedenheit des Patienten wird jedoch nicht nur durch eine erfolgreiche Wurzelkanalbehandlung, sondern auch durch die korrekte postendodontische Versorgung bestimmt. Durch die Trepanation kommt es zwangsweise zu einem relativ hohen Destruktionsgrad im Dentin und durch die daraus resultierende biomechanische Schwächung steigt die Frakturgefahr der restlichen Zahnhartsubstanz enorm. Im ungünstigsten Fall führt eine Vertikalfraktur zur Extraktion des Zahnes. Die postendodontische Restauration soll den restlichen Zahn stabilisieren, die vollständige Funktion des Zahnes wiederherstellen und bakteriendicht abschließen. Dem Patientenwunsch entsprechend entwickelt sich die Wissenschaft mehr zu ästhetisch anspruchsvolleren zahnfarbenen Versorgungen hin – Keramiken und Komposite gelten als Restaurationen der Wahl, doch auch Gold spielt immer noch eine Rolle. In der Literatur wurden viele unterschiedliche Konzepte zur post endodontischen Versorgung vorgestellt und empfohlen. Und dennoch zeigen sich immer wieder Unsicherheiten in der klinischen Umsetzung. Material und Methode In 96 extrahierte Weisheitszähne (Lagerung in destilliertem Wasser, n=8) wurden für 11 Versuchsreihen (und Kontrollgruppe) endodontische Behandlungen durchgeführt und verschiedene Kavitäten präpariert (mod-Füllungen, mod-Inlays und Teilkronen). Die Zähne wurden mit folgenden Materialien versorgt: Komposit direkt (Tetric EvoCeram Bulk-Fill, Ivoclar Vivadent), Keramik indirekt (IPS Empress, Ivoclar Vivadent; e.max CAD, Ivoclar Vivadent; Celtra Duo, Dentsply DeTrey; Lava Ultimate, 3M Espe) und Gold indirekt (Degussa). In 1.200.000 Zyklen á 50 N wurden die Proben im Kausimulator erst subkritisch und in einer Universal prüfmaschine schließlich kritisch bis zum Bruch belastet. Ergebnisse Die statistischen Berechnungen wurden mittels SPSS 15.0 für Windows durchgeführt. Folgende Testverfahren kamen für die Auswertung zum Einsatz: der Kolmogorov-Smirnov-Test, der Wilcoxon-Vorzeichen-Rang-Test und der Kruskal-Wallis-Test. Die höchsten Bruchfestigkeitswerte ergaben sich für die Versorgung mit Goldteilkronen sowie für die Kontrollgruppe mit den unbehandelten Zähnen (p<0,05). Die niedrigsten Werte wurden für IPS Empress-Inlays, IPS Empress-Teilkronen, Kompositfüllungen, e.max CAD-Inlays und Celtra Duo-Inlays gemessen (p<0,05). Auffällig war, dass bis auf IPS Empress die Werte für die Teilkronenversorgungen deutlich höher waren als die für die Inlayversorgungen. Schlussfolgerungen Die Ergebnisse dieser Untersuchung zeigten, dass ein signifikanter Unterschied bei den Bruchfestigkeitswerten der Inlay- und der Teilkronenversorgungen vorhanden war. Mit Ausnahme der IPS Empress-Restaurationen schnitten die Teilkronenversorgungen deutlich besser ab. Die höchsten Bruchfestigkeitswerte wurden für Goldteilkronen und die natürlichen Zähne ohne Präparation gemessen. Aufgrund der vorliegenden Ergebnisse stellt die postendodontische Restauration mit Teilkronen bezogen auf die Bruchfestigkeit die beste Versorgungsmöglichkeit dar. Bei dem Vergleich aller Versuchsgruppen fielen die sehr hohen Werte bei den Goldteilkronen Restaurationen auf. Insgesamt entsprachen die Ergebnisse der Empirie.
