Zusammenfassung:
Mit über 600 000 Todesfällen pro Jahr ist der Leberkrebs weltweit unter den 10 häufigsten Krebstodesursachen zu finden [59]. Der mit bis zu 85% häufigste histologische Subtyp ist das hepatozelluläre Karzinom, das somit bereits im Jahr 1990 für über 5% aller Krebsfälle weltweit verantwortlich war [12]. Das HCC ist mit einer 5-Jahres-Überlebensrate von 6,5% mit einer sehr schlechten Prognose assoziiert [25]. In den letzten Jahren fand sich eine stetige Zunahme der HCC-Inzidenz v.a. in Ländern mit geringer HCC-Prävalenz wie den USA und Westeuropa. Hingegen wurde in Ländern mit hoher HCC-Prävalenz, wie in Teilen Asiens und Afrikas, ein Rückgang der Neuerkrankungen verzeichnet [27]. Weltweit sind Männer etwa doppelt so häufig betroffen wie Frauen [40]. Als Hauptrisikofaktor für die Entstehung eines HCC gilt die Leberzirrhose, wobei weltweit v.a. die Infektion mit dem Hepatitis-C-Virus eine hohe Inzidenz an HCC-Erkrankungen bei Patienten mit Zirrhose aufweist [28]. Eine frühe Diagnosestellung ist bei den geringen Therapieoptionen für die Patienten prognosebestimmend.
Ziel dieser Dissertation war es, die aktuelle Studienlage zur Diagnostik des HCC darzustellen, vorhandene Studien in ihrer Durchführung und Qualität zu bewerten und ihren Evidenzgrad zu ermitteln, um dann die Ergebnisse in Form einer Metaanalyse quantitativ zusammenzufassen. Hierfür wurde auf PubMed eine umfassende und systematische Literaturrecherche durchgeführt, die so gefundenen Studien wurden auf vorher bestimmte Ein-und Ausschlusskriterien untersucht und ggf. ausgeschlossen. Die aufgenommenen Studien wurden von zwei Bewertern unabhängig voneinander einem Evidenzgrad zugeordnet. Bei Uneinigkeiten wurde durch Diskussion ein Konsens gefunden. Anschließend wurde eine Metaanalyse durchgeführt, die sowohl eine Gesamtanalyse als auch mehrere Subgruppenanalysen beinhaltete. Die diagnostischen Richtigkeiten der einzelnen Studien wurden hierbei in Form der bivariaten Analyse ausgewertet, worin der zweidimensionale Effekt von Sensitivität und Spezifität Berücksichtigung fand.
Insgesamt wurden 46 Studien in die Analyse aufgenommen, davon benutzten 8 Studien die CT als diagnostischen Test, 10 Studien verwendeten den Ultraschall, 11 Studien das MRT und 4 Studien den AFP-Wert. 13 der Studien verwendeten mehr als einen diagnostischen Test. Keiner der Studien konnte ein höherer Evidenzgrad als 2B zugeordnet werden. Häufige Gründe für die Herabstufung der Qualität der Studien waren fehlende Angaben über die Unabhängigkeit des Referenzstandards sowie über den Umgang mit nicht eindeutigen Befunden. Für die Ultraschalluntersuchung (7927 Patienten) konnte in der bivariaten Analyse ein Sensitivitätswert von 66,2% (KI 54,0% - 76,5%) bei einer zugehörigen Spezifität von 95,7% (KI 91,1% - 98,0%) gefunden werden. Für die klinische Praxis ist die kostengünstige und schnelle Untersuchung mit US bei befriedigender diagnostischer Richtigkeit somit v.a. für die Screeninguntersuchung geeignet. Die Analyse der Computertomographie (1578 Patienten) ergab für die Sensitivität 72,2% (KI 63,5% - 79,5%) und für die Spezifität 90,3% (KI 84,8% - 93,9%). In der Subgruppenanalyse zeigte sich kein Vorteil hinsichtlich Studien mit höherem Evidenzgrad oder Impaktfaktor, es zeigten sich aber auch hier gute Ergebnisse in den einzelnen Untergruppen. Für die Magnetresonanztomographie (882 Patienten) wurden in der bivariaten Analyse die besten Ergebnisse gefunden: Sensitivität 82% (KI 71,9% - 89%), Spezifität 90,3% (KI 83,5% - 94,4%). Bei einer mit der CT vergleichbaren Spezifität fand sich eine deutlich höhere Sensitivität. Die Subgruppenanalyse ergab gleich gute Ergebnisse in allen Untergruppen, jedoch fand sich auch hier keine Abweichung vom Gesamtergebnis. Die bivariate Analyse für die AFP-Diagnostik (2154 Patienten) ergab für die Sensitivität bei einem Cut-off >200 ng/ml einen Wert von 39,6% (KI 23,9% - 57,8%) und für die Spezifität einen Wert von 98,9% (KI 97,9% - 99,4%). Als sehr unspezifischer Marker bestätigte sich somit der relativ geringe Nutzen in der klinischen Praxis für die Diagnosestellung des HCC. Für die Diagnostik des hepatozellulären Karzinoms zeigen sich v.a. für die CT- und MRT- Untersuchungen gute diagnostische Richtigkeiten. Die Kosten- und Zeitintensivität dieser Untersuchungen, sowie die Strahlenbelastung im CT stellen allerdings Limitationen in der täglichen Anwendung dar. Für die Screeninguntersuchung von Risikopatienten gilt somit weiterhin der Ultraschall als wichtige Voraussetzung. Dieser kann in Kombination mit der Erhebung des AFP-Werts ebenfalls gute Ergebnisse erzielen. Für die genaue Vergleichbarkeit der unterschiedlichen Diagnoseverfahren bedarf es aber auch in Zukunft der vermehrten Durchführung qualitativ hochwertiger Diagnosestudien.
