Zusammenfassung:
Hintergrund Eine medikamentöse Therapie kann nur dann ihre Wirkung entfalten, wenn sie wie empfohlen eingenommen wird. Gründe für die Unterbrechung oder Beendigung einer Therapie durch den Patienten können unter anderem schlechte Arzt-Patient-Kommunikation, ungenügend medizinische Betreuung und durch die Therapie hervorgerufene Nebenwirkungen sein. Im Falle der endokrinen Therapie in der adjuvanten Situation des primären Mammakarzinoms gehen die schlechten Complianceraten mit einer erhöhten Mortalität einher. Die Nebenwirkungen im Sinne von Schlafstörungen, Verschlechterung der Lebensqualität, sowie Veränderungen des Knochenstoffwechsels scheinen einen Einfluss auf die Therapietreue zu haben. Methoden Die Untersuchung lief parallel zur COMPAS-Studie („Einfluss einer schriftlichen oder mündlichen Patientenerinnerung unter Alltagsbedingungen auf die Compliance hinsichtlich der Einnahme eines Aromatasehemmers in der adjuvanten Therapie des primären Mammakarzinoms im Vergleich zur Standardaufklärung, im Rahmen der Versorgungsforschung“), in welche 181 Patientinnen rekrutiert wurden, die aufgrund eines primären Mammakarzinoms eine endokrine Therapie mit einem Aromataseinhibitor erhielten. Die Patientinnen wurden in diesem monozentrischen, dreiarmigen, teilverblindeten, randomisierten Parallelgruppenvergleich über 12 Monate mit Serienbriefen oder Telefonaten begleitet und mit einer Kontrollgruppe in Hinblick auf die Parameter der Compliance verglichen. Im Rahmen des Addendums wurde die Hauptstudie bei Aufnahme, sowie nach 12 Monaten durch standardisierte Fragebögen zur Evaluierung von Nebenwirkungen, sowie einer Blutentnahme zur Untersuchung von Knochenstoffwechselmarkern ergänzt. Hiermit sollte gezeigt werden, ob sich ein Einfluss von möglichen Nebenwirkungen, wie Schlafstörungen, Veränderungen der Lebensqualität, sowie das Verhalten der Knochenstoffwechselmarker unter der endokrinen Therapie auf die Parameter der Compliance darstellen lässt. Ergebnisse In Hinblick auf die Lebensqualität zeigte sich kein signifikanter Unterschied zwischen compliant und non-compliant Patientinnen. Bei den Schlafstörungen wiesen die noncompliant Patientinnen einen signifikant höhere Rate lediglich im Bereich der Tagesschläfrigkeit auf. Im Bereich der Knochenstoffwechselparameter wurde nach 12 Monaten Therapie mit einerm Aromatasehemmer ein signifikanter Anstieg von CTX als Marker der Knochenresorption und tP1NP als Knochenformationsmarker beobachtet. Wobei die beiden Marker in der Gruppe der therapietreuen Patientinnen in sehr viel deutlicherem Ausmass anstiegen, als im non-cpmpliant Patientenkollektiv. Der ebenfalls untersuchte Serumestradiolspiegel wird wie zu erwarten bei den compliant-Patientinnen stärker supprimiert, als in der non-compliant Patientengruppe. Diskussion Auch in dieser Untersuchung konnte keine signifikante Einflussnahme der Aromatasehemmertherapie auf die Items der Lebensqualität nachgewiesen werden. Die Signifikanz hinsichtlich der Tagesschläfrigkeit im Bereich des Schlafverhaltens ist am ehesten zu vernachlässigen, da davon ausgegangen werden muss, dass bei klinischer Relevanz auch weitere den Nachtschlaf beeinträchtigende Faktoren Signifikanz aufzeigen würden. Die Untersuchungsergebnisse in Hinblick auf den Knochenstoffwechsel zeigen nach 12 Monaten Therapie mit einem Aromatasehemmstoff sowohl einen signifikanten Anstieg der Parameter im Bereich der Knochenresorption, als auch der Knochenformation, während frühere Untersuchungen einen Anstieg der Knochenformationsparameter nur unter der Therapie mit Exemestan aufwiesen. In diesen Studien wurde allerdings ein deutlich kürzerer Beobachtungszeitraum gewählt, so dass in Zukunft Untersuchungen mit einem längeren Beobachtungszeitraum den Auswirkungen der antihormonellen Therapie auf den Knochenstoffwechsel nachgehen sollten.
