Zusammenfassung:
Hintergrund: Das Cognitive Behavioral Analysis System of Psychotherapy (CBASP) ist eine Psychotherapiemethode zur Behandlung der chronischen Depression, die ursprünglich als Einzeltherapie für den ambulanten Rahmen entwickelt wurde. Darauf aufbauend entstand ein Gruppentherapiekonzept für die Anwendung im stationären Bereich. CBASP fußt auf einer Reihe von Grundannahmen über chronisch Depressive, die aus der langjährigen Arbeit mit dieser Patientengruppe erwuchsen. Eine dieser Annahmen besagt, dass chronisch depressive Patienten spezifische Entwicklungsdefizite im emotionalen Bereich aufweisen und sich nach dem Entwicklungsmodell von Piaget auf einem „präoperationalen Niveau“ befinden. Dies führt zu ineffektiven sozialen Interaktionen, Defiziten im Bereich interpersonaler Empathie sowie Schwierigkeiten in der Erkennung der eigenen Wirkung auf ein soziales Gegenüber (sog. Stimuluscharakter). Ziel vorliegender Studie war es daher, neben der Auswertung von Effektivität und Durchführbarkeit der neuen Gruppentherapie auch deren Einfluss auf präoperationale Strukturen, Empathie, Soziales Problemlösen und den Stimuluscharakter zu evaluieren. Methoden: Die CBASP-Gruppentherapie wurde in einer naturalistischen Multicenterstudie in den Studienzentren Bremen, Freiburg, Marburg und München untersucht. Um die weiterführende Hypothese zu testen, dass präoperationale Muster durch die Gruppentherapie beeinflussbar sind, untersuchten wir eine Stichprobe von n = 20 Patienten an der Universitätsklinik in Marburg mit zusätzlichen Instrumenten. Eingeschlossen wurden Patienten mit der primären Diagnose einer chronischen Depression, einer Dysthymie oder einer Double Depression (nach DSM IV). Die Durchführung der Studie wurde von der lokalen Ethikkommission genehmigt. Die Patienten nahmen im Durchschnitt an 19.1 +/- 8.2 CBASP Gruppensitzungen in einer Länge von 1,5 Stunden teil. In 80% der Fälle erhielten die Patienten zusätzlich CBASP-Einzeltherapie. Die folgenden Messinstrumente wurden vor und nach Gruppentherapie eingesetzt: Selbst- und Fremdbeurteilung der Depressivität mittels Becks Depression Inventory (BDI) und Hamilton Depression Rating Scale (HDRS), Erfassung von Lebensqualität durch den WHO-Bogen Quality of Life (WHOQOL-BREF), Fragebögen zu Zufriedenheit und Akzeptanz mit der Behandlung, Erfassung präoperationaler Denkstrukturen durch den Lübeck Questionnaire for recording Preoperational Thinking (LQPT), soziale Problemlösekompetenzen durch das Social Problem Solving Inventory (SPSI-R), Impact Message Inventory (IMI-R) and Reading the Mind in the Eyes Test (RMET-R). In einer Follow-up-Untersuchung wurde BDI und Lebensqualität drei Monate nach Therapieende gemessen. Zur Testung der Prä-/Post-Effekte kamen ANOVAs im Messwiederholungsdesign zur Anwendung. Zur Messung der Prädiktoreigenschaften der beeinflussten Defizite wurden hierarchische schrittweise Regressionsanalysen gerechnet. Ergebnisse: Analog zur Multicenterstudie zeigte sich bei den Patienten eine große Akzeptanz der Gruppentherapie. Im Prä-Post Vergleich zeigten sich hochsignifikante Verbesserungen der psychopathologischen Symptome (BDI -51.96%, HAMD -61.76%) sowie der subjektiven Lebensqualität (+24.91%). Die Abnahme präoperationaler Denkstrukturen hin zu formal-operationalen Denkweisen erwies sich ebenfalls als hochsignifikant (p=.003). Ebenso verhielt es sich mit den sozialen Problemlösefähigkeiten (+38.77%, p<.001). Auch der Stimuluscharakter konnte durch die Therapie verändert werden. Die Zunahme freundlich/dominanter sowie die Abnahme feindselig/submissiver Impact Messages zeigten dabei die besten Entwicklungen. Zusätzlich wurde eine signifikant höhere Anzahl der vorgelegten emotionalen Gesichtsausdrücke richtig gedeutet, was als steigendes Maß für Empathie gewertet wird (+12.03%, p=.007). In Regressionsanalysen konnte eine gute Prädiktoreigenschaft insbesondere von Veränderung des Denkniveaus sowie sozialer Problemlösekompetenzen über die Dauer der Therapie für das Therapieoutcome und die depressive Symptomatik drei Monate nach Therapieende gezeigt werden. Diskussion: Die vorliegende Studie liefert Hinweise, dass die multimodale Behandlung der chronischen Depression, die eine CBASP-Gruppentherapie beinhaltet, eine gute Verbesserung depressiver Symptome und subjektiver Lebensqualität bewirkt und bei den Patienten auf große Akzeptanz stößt. Darüber hinaus gibt es gute Anhaltspunkte, dass sich zudem ein therapeutischer Effekt auf eine Anzahl an spezifischen Defiziten chronisch depressiver Patienten nachweisen lässt. Die Patienten zeigen durch die Therapie eine Verbesserung ihres Denkniveaus hin zu rational logischem Denken, welches durch die Umwelt beeinflussbar ist, und sind zu effektiverem Problemlösen fähig. Die Entwicklung dieser Eigenschaften enthält einen prädiktiven Wert für die Vorhersage des Therapieerfolgs über das Ende der Therapie hinaus. Die Interpretation der Ergebnisse ist durch die geringe Stichprobengröße sowie die naturalistische Vorgehensweise mit Einbettung der Gruppentherapie in einen individuell bedarfsadaptierten, mulitimodalen Behandlungszusammenhang limitiert. Zudem entstanden durch die Erhebung ohne Vergleichsgruppe weitere Einschränkungen für die Interpretierbarkeit. Für die Überprüfung der gewonnenen Erkenntnisse sind größerangelegte, randomisiert kontrollierte Studien vonnöten.
