Zusammenfassung:
Glenohumerale Verletzungen sind häufige Begleiterscheinungen bei acromioclavicularer Dislokation (Synonym: Schultereckgelenksverletzung, AC-Gelenkssprengung, ACG-Luxation). Der zunehmende Einsatz minimal-invasiver Techniken erleichtert die Diagnose der Begleitpathologien und ermöglicht die einzeitige Therapie. Ziel der vorliegenden Arbeit ist es, die Inzidenz intraartikulärer Begleitverletzungen bei höhergradigen Schultereckgelenksverletzungen (Rockwood III – V) zu erheben und mögliche Risikokonstellationen zu identifizieren. Material und Methoden In den Jahren 2009 bis 2015 wurden 163 konsekutive Patienten (147 Männer, 16 Frauen, Durchschnittsalter: 36,8 Jahre) mit höhergradiger AC-Gelenkssprengung (Rockwood III: n = 60; Rockwood IV: n = 6; Rockwood V: n = 97) in der Unfallchirurgie des Universitätsklinikums Marburg im Rahmen der arthroskopisch-assistierten AC-Gelenksstabilisierung diagnostisch arthroskopiert und die intraartikulären Begleitpathologien erfasst. Diese wurden deskriptiv statistisch ausgearbeitet und anschließend kausal zugeordnet (degenerativ, intermediär, traumaassoziiert). Subgruppenanalysen bezüglich des Schweregrads der ACG-Luxation, des Geschlechts und des Patientenalters zum Zeitpunkt der OP wurden vorgenommen. Ergebnisse Männer zogen sich eine AC-Gelenksverletzung signifikant häufiger zu als Frauen (p < 0,001). –Verletzungen nach Rockwood Stadium V traten in diesem Patientenkollektiv signifikant häufiger auf (p = 0,015). Bei 39,3 % (n = 64) Patienten mit höhergradiger AC-Gelenkssprengung wurde mindestens eine Begleitpathologie diagnostiziert. Die Inzidenz von Verletzungen der Rotatorenmanschette lag bei 32,3 % (n = 40) der Begleitpathologien. Knorpelschäden beliefen sich auf 30,6 % (n = 38), SLAP-Läsionen auf 22,6 % (n = 28). 70,0 % (n = 49) der Begleitpathologien waren auf degeneratve Prozesse zurückzuführen. Bei 21,9 % (n = 14) der Patienten erforderte die Begleitverletzung ein rekonstruktives Verfahren. In den Subgruppenanalysen nach Geschlecht und Rockwood-Stadien zeigte sich kein Unterschied bezüglich der Ätiologie, Art und Häufigkeit des Auftretens von Begleitpathologien. In der Subgruppenanalyse der beiden Altersgruppen trat bei 57,3 % (n = 43) aller Patienten über 35 Jahre im Rahmen einer Dislokation im AC-Gelenk eine Begleitpathologie auf. Es bestand eine signifikante Assoziation zwischen höherem Alter und Inzidenz von Begleitpathologien jeglicher Art (p < 0,001). Spezifische Begleitpathologien bei der Subgruppe der Patienten über 35 Jahren waren die SLAP-Läsionen (p < 0,001), Läsionen der Supraspinatussehne (p < 0,001), Knorpelschäden am Humerus (p = 0,029) und Verletzungen der langen Bizepssehne (p = 0,002). In dieser Subgruppe zeigte sich außerdem ein signifikant vermehrtes Vorkommen von degenerativ bedingten Begleitpathologien (p < 0,001). Eine signifikant vermehrte Sportassoziation konnte bei den Patienten unter 35 Jahren festgetellt werden. In der Literatur wurde die Zahl an intraartikulären Begleitpathologien bei akuter Schultereckgelenksverletzung bis dato unterschätzt. Nahezu 40 % aller höhergradigen AC-Gelenksverletzungen wiesen mindestens eine glenohumerale Begleitpathologie auf. Die höchsten Inzidenzen von Begleitpathologien (57,3 %) traten insbesondere im Kollektiv der über 35-jährigen Patienten auf. Sie waren zu 70 % durch degenerative Vorschäden bedingt bzw. begünstigt. Hauptverletzungen waren Verletzungen der Rotatorenmanschette, SLAP-Läsionen und Knorpelschäden. Bei 8,6 % der Patienten bedurfte die Verletzung zwingend einer rekonstruktiven Therapie. Das Risikokollektiv sollte großzügig weiterer Diagnostik zur Feststellung der intraartikulären Begleitpathologien zugeführt werden, denn rein klinisch sind diese direkt posttraumatisch sehr unspezifisch und schwierig bzw. nicht diagnostizierbar. Bei bestehender Operationsindikation erlaubt die arthroskopisch assistierte Therapie der AC-Gelenkssprengung die beste Möglichkeit der Identifizierung der potentiellen Begleitpathologien und deren einzeitige Versorgung.
