Zusammenfassung:
ZUSAMMENFASSUNG Hintergrund und Problemstellung Die endodontische Behandlung spielt im Rahmen der konservierenden Zahnerhaltung eine wichtige Rolle. Sie zielt darauf, stark zerstörte und devitale Zähne nicht direkt extrahieren zu müssen, indem das nekrotische Gewebe entfernt, Keime eliminiert und der Zahn so weit aufbereitet wird, dass er eineWurzelkanalfüllung und gegebenenfalls, je nach Zerstörungsgrad, zusätzlich einen Stift aufnehmen kann. Der Stift dient als weitere Retention für die spätere Restauration. Oft kommt es vor, dass in akzessorischen Seitenkanälen des Wurzelkanals oder in apikalen Verzweigungen Restgewebe und Keime verbleiben, die zu einer Reinfektion des Wurzelkanals führen können und somit kein dauerhafter, beschwerdefreier Zustand des Zahnes bzw. der Halt des Stiftes gewährleistet werden kann. In diesem Sinne versucht man herauszufinden, inwieweit verschiedene Spüflüsigkeiten sich auf die Reinigung und Desinfektion des Wurzelkanalsystems auswirken und den Haftverbund des Sealers zum Wurzelkanaldentin beeinflussen. Des Weiteren stellt sich die Frage, ob der Smear layer, der bei der Aufbereitung entsteht, entfernt oder besser belassen werden sollte. Im Rahmen dieser Studie sollte anhand unterschiedlicher Spülprotokolle festgestellt werden, inwiefern verschiedene endodontische Spülflüssigkeiten den Haftverbund von AH 26 zum Wurzelkanaldentin beeinflussen. Material und Methode Zur Versuchsdurchführung wurden 60 extrahierte, einwurzelige, humane Zähne mit geradem Wurzelquerschnitt, die vorher noch nicht endodontisch behandelt wurden und keine größeren Defekte aufwiesen, ausgewählt. Nach Dekoronation wurden die Proben mittels BioRaCe (FKG Dentaire) maschinell bis ISO 60 und einer Arbeitslänge von 8 mm aufbereitet. Die Versuchsproben wurden in füf Gruppen (n=12) randomisiert aufgeteilt: Gruppe I: EDTA 17 % Gruppe II: NaOCl 3 % Gruppe III: CHX 2 % Gruppe IV: Zitronensäure 40 % Gruppe V: Aqua dest. (Kontrollgruppe) Die einzelnen Gruppen wurden mit 5 ml der jeweiligen Spülflüssigkeit für drei Minuten gespült. Nach Trocknung mittels Papierspitzen wurde AH 26 mithilfe eines Lentulo in den Wurzelkanal appliziert und ein standardisierter Stahlspreader (ISO 60) auf die entsprechende Arbeitslänge von 8 mm eingebracht. Die Endposition wurde kontrolliert und die Proben für zwei Wochen in einem Thermoschrank bei 37 °C gelagert. Die Analyse der Haftwerte des Sealers erfolgte anhand des Pullout-Tests nach Ebert et al.(2011) und der Zwick-Universaltestmaschine 1120 (Zwick-Roell). Die statistische Auswertung wurde mit SPSS 21 (IBM) unter Verwendung des Kruskal-Wallis- und des Mann-Whitney-Tests vorgenommen. Zur Bestimmung der Frakturmodi (adhäsiv, kohäsivoder gemischt) wurden die Spreader nach dem Herausziehen unter dem Lichtmikroskop von zwei Seiten fotografiert. Ergebnisse Nach der Spüung mit destilliertem Wasser wurden die höchsten Haftwerte (5,77 MPa)erreicht. Zitronensäure schnitt mit einem Mittelwert von 0,82 MPa im Vergleich mit allen anderen Lösungen signifikant am schlechtesten ab. Auch nach Verwendung von NaOCl wurde ein signifikant niedriger Haftverbund des AH 26 zum Wurzelkanaldentin gemessen als nach Spülung mit Aqua dest. Bei der Analyse der Spreader wurde weitestgehend ein gemischter Frakturmodus beobachtet. Schlussfolgerung Die Ergebnisse dieser Studie zeigen auf, wie unterschiedlich die gemessenen Haftwerte nach Spüung mit den verschiedenen Spülflüssigkeiten ausfallen. Bei der Spüung mit Zitronensäure wurden signifikant schlechtere Haftwerte ermittelt. Dies könnte an einer zu starken Demineralisation des Wurzelkanaldentins liegen, da es sich bei der Spüflüsigkeit um eine starke Säure handelt. Um den Smear layer zu entfernen, sollte daher der Spülung mit EDTA eindeutig der Vorzug vor Zitronensäure gegeben werden. Es bedarf weiterer Studien, inwiefern sich die jeweiligen Kombinationen der einzelnen Spüflüsigkeiten auf den Haftverbund des Sealers auswirken.
