Zusammenfassung:
Die chronisch obstruktive Lungenerkrankung COPD kann trotz ihrer hohen sozioökonomischen Relevanz als komplexes und in mehrfacher Hinsicht bei weitem nicht vollständig verstandenes Krankheitsbild angesehen werden. Ein wichtiger Aspekt im Krankheitsverlauf der COPD sind akute Exazerbationen. Sie tragen erheblich zur Morbidität und Mortalität der COPD bei und sind wie die COPD selbst heterogen in Bezug auf Ursachen, Ausprägung, Frequenz und Auswirkung auf den individuellen Krankheitsverlauf. Neben etablierten Funktionstests kann die Computertomographie unter anderem Hinweise für die dominante Ausprägungsform der COPD geben und darüber hinaus wertvolle Zusatzinformationen, etwa zu Verteilungsmuster und Heterogenität der Veränderungen liefern. Hauptziel dieser Arbeit war, herauszufinden ob sich im CT Messparameter finden lassen, die einen unabhängigen Prädiktor für die Exazerbationshäufigkeit der COPD darstellen. Daneben sollten im CT gewonnene Messparameter auch auf mögliche Zusammenhänge mit Messwerten etablierter Funktionstests untersucht werden. Dazu wurden im Zeitraum 2010 - 2011 retrospektiv 43 Patienten mit der Diagnose COPD erfasst, die im gleichen Klinikumsaufenthalt sowohl eine Multislice-CT als auch mindestens einen Funktionstest (Ganzkörperplethysmographie, 6-minute-walk-test, Blutgasanalyse) erhalten hatten. Für alle Patienten wurden zwischen 2006 und 2014 dokumentierte Exazerbationsereignisse erfasst. In der CT wurden mittels der Prototyp-Software Mevis Pulmo 3D global und Lungenlappen-basiert Messwerte der Lungendichte akquiriert (unter anderem der Emphysemindex LAV2, die mittlere Lungendichte MLD und der Scheitelpunkt des Dichtewerthistogramms PK). Aus den Lappen-basierten Messungen jedes Datensatzes wurden Maximum und Minimum bestimmt und deren Differenz als Maß der Emphysemheterogenität ermittelt. Als Korrelat der Bronchusobstruktion im CT wurde die Bronchuswanddicke semiautomatisch mittels der Software Airway Inspector ermittelt und daraus der PI10-Wert, die Wanddicke eines idealisierten Bronchus mit 10 mm Innenumfang, gebildet. Als wichtigste Messparameter der Lungenfunktionsdiagnostik mittels Ganzkörperplethysmographie wurden die absolute und normalisierte Einsekundenkapazität LF_FEV1 bzw. LF_FEV1%Soll sowie der Atemwegswiderstand LF_Rspez bzw. LF_Rspez%Soll verwandt. Hauptmessparameter des 6 Minute Walk Test war die zurückgelegte Gehstrecke 6MWT_Strecke. Mit Bezug auf das Hauptzielkriterium der Arbeit konnte mittels schrittweiser Regression ein Zusammenhang der Exazerbationsfrequenz ExFreq mit der globalen Emphysemausprägung in Form des Emphysemindex LAV2 (p=0,014) sowie mit der Emphysemheterogenität gemessen anhand der Spitze des Dichtewerthistogramms het_PK (p=0,002) gezeigt werden. Ein solcher Zusammenhang wurde bisher nur in vereinzelten Arbeiten für den Emphysemindex untersucht, wobei er nicht durchgängig bestätigt werden konnte. Arbeiten, die einen Einfluss der Emphysemheterogenität auf die Exazerbationsfrequenz untersuchten, waren in der Literaturrecherche gar nicht auffindbar, so dass dieses Untersuchungsergebnis zunächst einer Bestätigung in weiteren Tests bedarf. In den Korrelationstests konnte eine signifikante inverse Korrelation des im CT ermittelten Emphysemindex LAV2 mit der Einsekundenkapazität LF_FEV1 gefunden werden, ein Ergebnis das durch viele weitere Studien in ähnlicher Deutlichkeit gestützt wird. Die Emphysemheterogenität korrelierte für einzelne Marker negativ mit der Einsekundenkapazität und positiv mit dem Atemwegswiderstand; andere Heterogenitätsmarker zeigten keine solche Korrelation, so dass ein eindeutiger Zusammenhang in dieser Arbeit nicht gesichert werden konnte. Deutlich zeigte sich eine inverse Korrelation der Emphysemheterogenität mit der Sechsminutengehstrecke. Studien, welche die Heterogenität des Lungenemphysems bei COPD und deren Auswirkungen untersuchen, waren im Vergleich zu Untersuchungen zur globalen Emphysemausprägung nur sehr selten und mit vergleichsweise kleinen Fallzahlen zu finden, bestätigen insgesamt aber den in dieser Arbeit gefundenen Trend. Eine signifikante Korrelation der im CT ermittelten Bronchuswanddicke mit Messwerten der konventionellen Funktionstests wurde nicht gefunden, was durchaus in Kontrast zu Ergebnissen mehrerer anderer Studien steht. Allerdings erscheint die Messung der Bronchuswanddicke im CT als ungleich schwieriger, fehleranfälliger und (zumindest mit der verwandten Software) deutlich weniger ausgereift im Vergleich zur Erfassung des Lungenemphysems. Die in dieser Arbeit verwandte Prototyp-Software zur Emphysemquantifizierung konnte zeigen, dass sie verlässliche, plausible und reproduzierbare Ergebnisse liefern kann, auf deren Basis ein besseres Verständnis der Veränderungen bei einer COPD und deren Auswirkungen auf den Krankheitsverlauf zu erwarten ist.
