Zusammenfassung:
Ziel der Studie Das Hauptziel der vorliegenden in vitro-Studie bestand darin, die Auswirkung eines Eugenol-haltigen provisorischen Restaurationsmaterials auf die adhäsive Verbundfestigkeit eines Vier-Schritt-Etch-and-rinse-Systems nach einer Anwendung standardisierter Reinigungsmethoden im Mikrozugfestigkeitsversuch zu untersuchen. Material und Methode 45 extrahierte humane kariesfreie Weisheitszähne wurden mit einer Klasse-I-Kavität versehen und randomisiert in zwei Hauptgruppen aufgeteilt, welche entweder mit Eugenol-haltigem provisorischem Zement (Temp-Bond®, Kerr Corporation, Orange, CA, USA) oder mit Eugenol-freiem Zement (Temp-Bond® NE™, Kerr Corporation, Orange, CA, USA) gefüllt wurden. Nach einer siebentägigen Lagerung in physiologischer Kochsalzlösung wurden die Kavitäten auf drei unterschiedliche Arten von den Zementrückständen gesäubert: Scaler/Excavator, Aluminiumoxid (RONDOflex Plus Powder, KaVo Dental GmbH, Biberach/Riß, Deutschland) und Glycinpulver (Clinpro™ Prophy Powder ®, 3M™ ESPE™ AG, Seefeld, Deutschland). In sämtlichen Versuchsreihen wurden die Dentinoberflächen mit einem Dentinhaftvermittler vorbehandelt (Syntac®, Ivoclar Vivadent AG, Ellwangen, Deutschland) und die Restauration mit einem Nanohybridkomposit (Venus® Diamond, Heraeus Kulzer GmbH, Hanau) in Inkrementschichttechnik gefüllt. Zur Ermittlung der Haftwerte wurden die Proben in Stäbchen mit einem Querschnitt von jeweils 0,7 mm² gesägt und einer Microtensile-Untersuchung (microtensile bond strength, µTBS) mit einer konstanten Vorschubgeschwindigkeit von 1 mm/min unterzogen. Die Oberflächenmorphologie und Adhäsiv-Dentin-Schnittstellen wurden in der Folge an einem Rasterelektronenmikroskop untersucht. Ergebnisse und Beobachtungen Ein Einfluss von Eugenol auf die Adhäsion von Dentin kann unter den Hauptgruppen nicht signifikant nachgewiesen werden (p > 0,05): Die Auswertung des Mikrozugfestigkeitsversuchs zeigt lediglich geringfügige Differenzen untereinander und im Vergleich mit der Kontrollgruppe. Die Mittelwerte der erzielten Adhäsionskräfte variierten zwischen 7,1 MPa und 18,4 MPa. In dieser Studie wurden jedoch signifikante Unterschiede zwischen denjenigen Gruppen erhalten, bei denen eine Kavitäten-Reinigung nach Zementapplikation mittels Pulverstrahlgerätes stattgefunden hatte (UG 2,3,6 und 7) und denen, die durch Scaler und Excavator gereinigt wurden (UG 1,4,5 und 8) (p < 0,05). UG 1 und 5 (8,2; 7,1 MPa) erreichten die geringsten Haftfestigkeiten, gefolgt von UG 4 und 8 (14,9; 15,4 MPa). Eine Vorbehandlung des Dentins mit Aluminiumoxid konnte innerhalb der Hauptgruppe die stärksten Ergebnisse in der Microtensile- Messung erreichen 16,4 MPa und 18,4 MPa. Dagegen bewirkt das vorherige Sandstrahlen mit Glycinpulver nur minimal niedrigere Haftfestigkeitswerte (15,2 MPa und 17,6 MPa). Praktische Schlussfolgerung Die Vorbehandlung von provisorischem Zementierungsmaterial, unabhängig davon, ob Eugenol-haltig oder Eugenol-frei, beeinflusst den adhäsiven Verbund zwischen Dentin und einem Nanohybridkomposit nicht, sofern seine Überreste durch Sandstrahlen entfernt werden. Die vorliegende Studie zeigt, dass ein manuelles Reinigungsverfahren durch Scaler und Excavator vermieden werden sollte, wenn die definitive Befestigung adhäsiv an das Dentin gebunden wird. Dabei stellt das Sandstrahlen mit Aluminiumoxid eine effiziente Möglichkeit dar; Glycin-Pulver kann als Alternative verwendet werden. Darüber hinaus ist eine wirksame Oberflächenreinigung entscheidend, um eine optimale Adhäsionskraft von Dentin am Komposit zu erreichen.
