Zusammenfassung:
Hautkrankheiten stellen einen häufigen Beratungsanlass in der Hausarztpraxis dar. Ziel der Studie war es, Zugangswege in das offizielle Gesundheitssystem und Krankheitserfahrungen von Patienten mit chronischen Hauterkrankungen zu analysieren. Wir führten semistrukturierte Interviews mit 16 Patienten aus städtischen sowie ländlichen Gebieten durch, die 23–38 Minuten dauerten. Die Patienten wurden über ihre Hausärzte rekrutiert. Die Interviews wurden aufgenommen, verbatim transkribiert und qualitativ von zwei unabhängigen Untersuchern nach Erstellen eines Codierungsbaumes inhaltsanalytisch ausgewertet. Zusätzlich wurde ein Fragebogen (n = 265) mit sieben Items in den Wartezimmern von drei Hausarztpraxen verteilt. Ergebnisse: Laieninformationen spielen eine wichtige Rolle bei der Beurteilung neu aufgetretener Hauterkrankungen. Durch die Krankheit entstandener Leidensdruck und von außen sichtbare Veränderungen geben den Anlass für einen Arztbesuch. Die Mehrzahl der Befragten würde hierbei zunächst ihren Hausarzt und nicht direkt einen Dermatologen aufsuchen. Die Krankheitsauswirkungen sind vielschichtig und betreffen soziale, berufliche, psychische Bereiche. Krankheitsmanagement im hausärztlichen Kontext ist sehr variabel, Krankheitsverhalten und Copingstrategien von Patienten sind interindividuell sehr unterschiedlich. Der Hausarzt ist in unserer Erhebung für die meisten Patienten mit Hauterkrankungen der gewünschte erste Ansprechpartner innerhalb des offiziellen Gesundheitssystems. Weitere Studien zur dermatologischen Versorgung hausärztlicher Patienten wären zu begrüßen.
