Zusammenfassung:
Nahezu die Hälfte der abgegebenen Medikamente in deutschen Apotheken entfällt auf rezeptfreie Arzneimittel (Over-The-Counter (OTC)-Arzneimittel). Der Großteil davon wird ohne Verschreibung abgegeben und ist somit der Selbstmedikation zuzuordnen, was den hohen Stellenwert des Selbstmedikationssektors in einer modernen Gesundheitsversorgung anzeigt. Hier kommt dem Apotheker eine zentrale Funktion zu. Im Rahmen des Beratungsgesprächs leistet er einen unverzichtbaren Beitrag, da er Möglichkeiten und Grenzen der Selbstbehandlung für den individuellen Patienten einschätzen und die adäquate therapeutische Maßnahme empfehlen kann. Dabei ergibt sich unausweichlich die Frage, auf welcher Grundlage der Apotheker OTC-Arzneimittel auswählt und empfiehlt. Um dem Anspruch der Wissenschaftlichkeit gerecht zu werden und eine stete Optimierung der Arzneimittelversorgung und Therapiesicherheit zu gewährleisten, muss die Auswahl eines OTC-Arzneimittels auf Grundlage einer differenzierten, wissenschaftlich fundierten Bewertung erfolgen. Ein Überblick über die Forschungsliteratur in diesem Bereich ergab, dass bislang kein geeignetes, wissenschaftlich begründetes, methodisch klar strukturiertes Instrument zur vergleichenden Einordnung und Bewertung der genannten Arzneimittel verfügbar ist. Das primäre Ziel der vorliegenden Arbeit war, ein wissenschaftsbasiertes, unabhängiges und transparentes Bewertungssystem für OTC-Arzneimittel zu erstellen. Eine geeignete Visualisierung sollte dem Apotheker eine eingängige, überschaubare Auswertung der Ergebnisse ermöglichen. Als erstes Anwendungsbeispiel wurde der akute, produktive Husten im Rahmen von viralen Infektionen der Atemwege (Erkältungshusten) ausgewählt. Beim zweiten Anwendungsbeispiel stand der akute, trockene Husten im Fokus. Gewählt wurde die Symptomatik aufgrund der außerordentlichen Praxisrelevanz in der Selbstmedikation, die mit einer hohen Beratungsaktivität des Apothekers verbunden ist. Im ersten Schritt wurde eine Status-quo-Analyse zur Bewertung von OTC-Arzneimitteln in Deutschland erhoben. Grundlage war eine Fragebogenerhebung, in der die Bewertungs-methoden und -kriterien von elf Institutionen und wissenschaftlichen Verlagen abgefragt wurden. Die rückläufigen Evaluationsbögen wurden eingehend ausgewertet, um Bewertungsmethoden und/oder -kriterien zu selektieren, die für die anschließende Entwicklung eines Bewertungssystems geeignet waren. Unter Berücksichtigung der Rückmeldungen der Status-quo-Analyse wurde im zweiten Schritt die konkrete Erstellung eines Bewertungssystems anhand der definierten Anwendungsbeispiele vollzogen. Bedingt durch die Vielzahl der unterschiedlichen Kriterien und Evidenzgrade stellte die Thematik ein ausgesprochen komplexes sowie zunächst unstrukturiertes Entscheidungsproblem dar. Aufgrund dessen wurde diese Problemstellung durch den Analytischen Hierarchieprozess (AHP) unterstützt. Der AHP ist ein Verfahren, das zur Entscheidungsunterstützung beziehungsweise -findung bei multifaktoriellen, vielschichtigen Entscheidungsproblemen eingesetzt wird. Diese Methode dient zur Auswahl einer möglichst optimalen, nachvollziehbaren Lösungsalternative unter Berücksichtigung aller relevanten Entscheidungsaspekte. Die selektierten chemisch-synthetischen (Acetylcystein, Ambroxol, Bromhexin, Dextromethorphan, Pentoxyverin) und pflanzlichen OTC-Arzneimittel (Cineol, Efeu-, Pelargonium sidoides-, Thymian-, Eibisch-Extrakt) wurden auf Grundlage der prädeterminierten Einflussgrößen unerwünschte Arzneimittelwirkungen (UAW) und Wirksamkeit untersucht, verglichen und bewertet. Die UAWs wurden über die Auswertung der Fachinformationen der Arzneimittel in der aktuellen Überarbeitung analysiert. Zum Nachweis der Wirksamkeit wurde eine breit angelegte systematische Literaturrecherche gemäß vorab definierter In- und Exklusionskriterien durchgeführt. Für die Auswertung wurden die Evidenzgrade I-III berücksichtigt. Gemäß dem Grundsatz der Beachtung der bestverfügbaren Evidenz wurde Evidenzgrad IV herangezogen, wenn keine Evidenz des Grades I-III vorlag. Weiterhin wurden relevante deutsche Leitlinien zur Bewertung der Wirksamkeit hinzugezogen. Abschließend wurden die nach Auswertung der Fachinformationen, Evidenzgrade und Leitlinien erhaltenen und auf den AHP angewendeten Ergebnisse in einer zusammenfassenden Übersicht visualisiert. Es wurde eine allgemein verständliche farbkodierte Darstellung analog zum Ampelschema verwendet. Den Kategorien A (grün), B (gelb) und C (rot) des Schemas wurden Intervalle zugeordnet, sodass eine Einordnung der erhaltenen Gesamtwerte der Arzneimittel in die einzelnen Kategorien möglich war. Die Gesamtergebnisse beider Anwendungsbeispiele wurden letztlich in einer Darstellung und separat für die Kriterien UAW und Wirksamkeit präsentiert. Die Analyse der UAWs zeigte, dass es sich bei den untersuchten Arzneimitteln weitgehend um sichere, gut verträgliche Produkte handelt. In Bezug auf das Ampelschema konnten alle pflanzlichen Arzneimittel Kategorie A zugeordnet sowie die chemisch-synthetischen Präparate in Kategorie B eingeteilt werden. Die Literaturrecherche ergab, dass für den Großteil der untersuchten Hustentherapeutika keine Evidenz für eine Wirksamkeit vorliegt. Ausnahmen stellen Pelargonium sidoides-Extrakt und in sehr begrenztem Umfang Cineol, Dextromethorphan und Eibisch-Extrakt dar. Zu Pelargonium sidoides-Extrakt wurden acht Studien für eine Auswertung als geeignet eingestuft. Sieben der acht Studien fielen als randomisierte, placebokontrollierte Studien unter Evidenzgrad Ib, eine Studie wurde als prospektiv durchgeführte Kohortenstudie unter Evidenzgrad III aufgeführt. Zwei der acht Studien wurden als Dosisfindungsstudien mit Tabletten durchgeführt, die weiteren sechs für die Darreichungsform Tropfen. Für Cineol wurde eine Studie des Evidenzgrades Ib und eine Studie des Evidenzgrades III heran-gezogen. Hinsichtlich Dextromethorphan wurde eine Studie des Evidenzgrades Ia und eine Studie des Evidenzgrades Ib eingeschlossen. Zur Wirksamkeit von Eibisch-Extrakt konnte eine Studie des Evidenzgrades III einbezogen werden. Da bezüglich der Wirksamkeit von Acetylcystein, Ambroxol, Bromhexin, Efeu-, Thymian-Extrakt sowie Pentoxyverin keine Evidenz des Grades I-III vorliegt, wurde Evidenzgrad IV berücksichtigt. Die einbezogenen Publikationen, aber auch die erfassten Leitlinien, kamen in der Regel hinsichtlich der mangelnden Evidenz für die Wirksamkeit zu einem ähnlichen Resultat. Pelargonium sidoides-Extrakt Tropfen konnten in Kategorie B eingeordnet werden. Alle anderen Arzneimittel erhielten eine Zuordnung zur Kategorie C des Ampelschemas.
