Zusammenfassung:
Verletzungen des Bandapparates zählen zu den häufigsten Sportverletzungen weltweit. Im Bereich des Kniegelenks gehört das vordere Kreuzband (VKB), mit einem Anteil von ca. 50%, zu den am öftesten rupturierten Bändern. Alleine in den USA werden jährlich über 100.000 VKB-Rupturen operativ versorgt. Die Integration des Sehnentransplantats in den Knochen stellt eine wichtige Voraussetzung für den Heilungserfolg dar und ist häufig mit Komplikationen verbunden. Bone Morphogenetic Proteins (BMPs) sind Gegenstand intensiver wissenschaftlicher Diskussionen. Die osteoinduktiven Effekte dieser Proteine werden häufig beschrieben und sind in zahlreichen experimentellen Studien vertreten. Groß angelegte in vitro- Studien für eine Verwendung dieser Proteine im Bereich der Kreuzbandchirurgie bestehen zum aktuellen Zeitpunkt allerdings nicht. Ziel unserer Untersuchungen war es zu zeigen, welchen Einfluss BMP-2 auf den Prozess der Knochen-Sehnen-Integration hat und inwiefern die externe Applikation von BMP-2 zu einer verbesserten Kalzifizierung von Sehnen führt. Hierzu wurde zunächst ein in vitro-Zellkultur-Modell auf Basis einer Knochen- Stammzellkultur etabliert. Mit Hilfe dieses Modells konnten bovine Sehnen über einen Versuchszeitraum von 3 Monaten kultiviert werden. Zur Messung des Kalzifizierungsgrades wurden zum einen mikroskopische, zum anderen histologische und biochemische Analysen vorgenommen. Unser Modell liefert ideale Bedingungen für Zellkulturuntersuchungen über einen langen Zeitraum. Mittels der etablierten Auswertmethoden lassen sich Kalzifizierungsprozesse zu unterschiedlichsten Zeitpunkten (Frühphase: biochemische Analytbestimmung, Spätphase: Histologie) analysieren. Sowohl Kultivierungszeit, als auch Populationsgröße sind beliebig erweiterbar. In unseren Versuchen haben wir Hinweise über den triggernden Effekt von BMP-2 auf die Mineralisationsvorgänge der Sehne erhalten. BMP-2 scheint den Prozess der Knochen-Sehnen-Integration zu beschleunigen und dadurch zu verbessern. Hierbei spielt neben dem, schon häufiger beschriebenen, Einfluss von BMP-2 auf Osteoblasten, wohl auch eine direkte Wirkung auf die Gruppe der Fibroblasten ein Rolle. Der genaue Mechanismus ist jedoch noch nicht verstanden. Wir vermuten aufgrund unserer Ergebnisse, dass BMP-2 eine Ossifikation fördert, die sowohl von den Osteoblasten, als auch von den Fibroblasten getragen wird. Im Prozess der Knochen-Sehnen-Integration könnte der Sehne somit ein bedeutender Anteil zukommen, sodass diese möglicherweise aktiv in den Knochen einwächst. Abschließend lässt sich feststellen, dass durch unsere Untersuchungen bereits bekannte Eigenschaften des BMP-2 bestätigt wurden und zusätzlich neue Effekte im Wirkspektrum dieser Proteinklasse aufgezeigt werden konnten. Weitere Untersuchungen in den angesprochenen Bereichen sind notwendig, um das Potential von BMP-2 vollständig auszuschöpfen.
