Zusammenfassung:
Das allergische Asthma bronchiale ist eine komplexe Erkrankung der Lunge, die mit einer bronchialen chronischen Hyperreagibilität, Entzündungsreaktion der Atemwegsobstruktion Atemwege, sowie einer strukturellen Veränderungen der Lunge im Sinne eines Airway remodelling assoziiert ist. Eine zentrale Komponente innerhalb der Pathogenese des allergischen Asthmas bildet die gestörte Homöostase zwischen TH1 und TH2 Lymphozyten mit einer vorherrschenden TH2-Immunantwort. Dabei führt die Ausschüttung von TH2-typischen Zytokinen, wie IL-4, IL-5, IL-13 und GM-CSF nach Allergen- Exposition zur Aktivierung und Rekrutierung von Eosinophilen Granulozyten in die Lunge, zur Mastzelldegranulation und einem Switch der Antikörperproduktion zur Synthese von IgE durch B-Zellen. Epidemiologische Untersuchungen führten zu der Überlegung, dass die niedrigere Infektionsrate sowie die geringere Exposition mit bakteriellen Bestandteilen in industrialisierten Ländern, die Ursache für die Polarisation in Richtung TH2 darstellt. Die Schaffung eines Milieus, in dem durch die selektive Aktivierung der TH1-Immunantwort die Entwicklung von TH2-Zellen unterdrückt wird, stellt aus diesen Gründen die Basis für die Entwicklung neuartiger Behandlungsstrategien für allergisches Asthma bronchiale dar. Die Entwicklung und Regulation der Effektorfunktionen von TH-Zellen steht unter enger Kontrolle der angeborenen Immunantwort, vornehmlich durch Bindung hochkonservierter Pathogen-assoziierter Muster (PAMPs) an Toll-like Rezeptoren (TLR) auf verschiedenen Immunzellen, aber auch Strukturzellen, was letztendlich zur Aktivierung dieser Zellen und Ausschüttung verschiedener Mediatoren führt. So wurde beispielweise gezeigt, dass variable TLR-Liganden wie bakterielle Bestandteile die Produktion von IL-12 in dendritischen Zellen stimulieren und dadurch die TH2-Antwort in Maus-Modellen für allergisches Asthma effektiv unterdrückt wird. Es ist seit langem bekannt, dass eine inverse Beziehung zwischen viralen Infektionen, wie Hepatitis A oder Masern, und der Etablierung eines allergischen Asthma bronchiale besteht. Dies führt zu der Hypothese, dass die Aktivierung von TLR3 beziehungsweise TLR7 durch synthetische Liganden den TH2-vermittelten allergischen Phänotypen unterdrückt. In dieser Arbeit wurde der Effekt der synthetischen TLR-Liganden Poly (I:C) (TLR3) und R-848 (TLR7) auf den allergischen Phänotypen im Maus-Modell für chronisches Asthma bronchiale untersucht. Im Gegensatz zu vorherigen Untersuchungen, die ausschließlich die Effekte von Poly (I:C) und R-848 auf die Etablierung beziehungsweise Pathogenese von akutem allergischem Asthma, wurden im Rahmen dieser Arbeit erstmals die Effekte auf chronische Entzündungsprozesse und Komponenten des Atemwegsumbaus untersucht. Es konnte gezeigt werden, dass sowohl die Applikation von R-848 als auch Poly (I:C) zur Abschwächung der TH2-vermittelten Immunantwort führt, so sank die Konzentration an OVA-spezifischem IgE sowie IL-5, während die Konzentration von IL-12 deutlich anstieg. Darüber hinaus kam es zu einer deutlichen Reduzierung der eosinophilen Granulozyten in der BAL. Überdies induzierte R-848, nicht jedoch Poly (I:C), eine verstärkte Synthese von OVA-spezifischem IgG2a, was der Aktivierung einer TH1-Antwort entspricht. Diese positiven Effekte waren auch hinsichtlich des Atemwegsumbaus nachweisbar. So waren in den peripheren Atemwegen im Vergleich zur OVA- Kontrollgruppe weit weniger mukusproduzierende Becherzellen vorhanden und es konnte weder eine Verdickung Atemwegsepithelien noch eine Verdickung der α-SMA-positiven Zellschicht ist in diesen Gruppen nachgewiesen werden. Entsprechend kam es zu einer deutlichen Verbesserung der Lungenfunktion nach Behandlung mit Poly (I:C) oder R-848. Zusammenfassend zeigen diese Ergebnisse, dass Anwendung synthetischer TLR3- und TLR7-Liganden im Rahmen eines Asthma bronchiale eine viel versprechende Therapieoption darstellt, die direkt in den zugrundeliegenden Mechanismus einer TH2-vermittelten allergischen Entzündung eingreift und durch Verschiebung der TH1-TH2-Balance in Richtung TH1 zahlreiche Charakteristika eines chronischen allergischen Asthma positiv beeinflusst.
