Zusammenfassung:
Scaffoldbasiertes Tissue Engineering von Knochen ist ein viel versprechender Ansatz zur Regeneration von kritisch großen Knochendefekten durch Trauma, Tumoren und Fehlbildungen sowie bei der Behandlung von Pseudarthrosen. Von der Entwicklung der Präparate zur klinischen Anwendung geeigneten Scaffolds ist die in vivo Testung der Implantate ein entscheidender Schritt.
Im Rahmen unseres Projektes wurde ein Composite-Scaffold aus dem Polymer Polylactid-co-Glycolid-Acid (PLGA) und Kalziumphosphat entwickelt und eingesetzt, mit allogenen mesenchymalen Stammzellen (MSC) im Festbett-Perfusions-Bioreaktor besiedelt oder unbesiedelt im Tierversuch erprobt. Als Tierversuchsmodell wurde das Konzept des „Critical size defects“ (=Defekt kritischer Größe) Im Femur des Chinchilla Bastard Kaninchens zu Grunde gelegt.
Um möglichst viele Informationen aus diesen ersten „klinischen“ Versuchen zu gewinnen, war eine möglichst Aussagekräftige Untersuchungen notwendig. Wir haben die neue, im Tissue Engineering von Knochen bisher kaum angewandte flat panel volumetric Computer Tomography (fpvCT) zur in vivo Evaluation eines Kleintierversuches getestet und mit anderen Methoden (Projektionsröntgen, Mikro-CT, Histologie) verglichen. Zentrale Fragen waren, ob die einliegende Osteosynthese die Darstellbarkeit verminderte (Artefaktbildung), das Scaffold mittels fpvCT darstellbar war und ob Degradation und Knochenwachstum im Verlauf beobachtet werden konnten, die genaue Evalutation des Wachstumsverhaltens, ob Gefäßneubildung im Osteotomiespalt gezeigt werden konnte und welche Rückschlüsse sich auf Tiermodell und Osteosynthese ziehen ließen.
Wir arbeiteten mit einem CT der Firma GE Global Research, Niskayuna, New York. Dieser flat panel volumetric Computed Tomograph arbeitet mit zwei flat panel Röntgensensoren mit einer Auflösung von jeweils 1024x1024 Pixeln.
Wir konnten zeigen, dass das fpvCT eine ernst zu nehmende Alternative in der in vivo Evaluation von Kleintierversuchen zum Scaffoldbasiertem Tissue engineering ist. Es ist dem Projektionsröntgen überlegen und kann das Mikro-CT – wenn die hohe Auflösung nicht benötigt wird – ersetzen. Große Vorteile dieser Methode gegenüber dem Mikro-CT ist die kürzere Scanzeit, die geringere Strahlenbelastung, der größere darstellbare Bereich und die Möglichkeit mehrerer Untersuchungen im Zeitverlauf an einem Tier durchzuführen. In der Auflösung ist das fpvCT dem Mikro-CT unterlegen. Vor allem bei Fragestellungen zum Aufbau des neu gewachsenen Knochens und der Differenzierung zw. degradiertem Scaffold und neuem Knochen ist die Histologie unerlässlich.
