Zusammenfassung:
Follikuläre dendritische Zellen (FDCs) bilden die Grundstruktur der Follikel in
allen sekundären lymphatischen Organen. Sie präsentieren langfristig auf ihren
Fortsätzen gebundene Immunkomplexe und spielen eine wichtige Rolle für die
Affinitätsreifung und den Isotypwechsel des Oberflächenimmunglobulins auf
Keimzentrums-B-Lymphozyten. Durch die Freisetzung von pro- und antiapoptotischen Mediatoren tragen sie darüber hinaus zur Selektion von BLymphozyten bei. Obwohl FDCs im gesamten Follikel vorkommen, differieren
ihre Oberflächenantigene in verschiedenen Regionen von Keimzentren, so dass
sich bei Menschen und Nagetieren FDC-Subpopulationen definieren lassen.
In der vorliegenden Arbeit werden FDCs durch immunhistologischen Nachweis
von CD21, CD35, CD23 sowie durch Nachweis der Bindung des monoklonalen
Antikörpers CNA.42 auf Paraffin-Serienschnitten von humanen Milz- und
Tonsillenpräparaten näher charakterisiert. Darüber hinaus wird eine bisher nicht
bekannte Expression der Antigene außerhalb des lymphatischen Gewebes beschrieben. Der Vergleich der Färbungen ermöglicht es, innerhalb eines
Follikels phänotypisch unterschiedliche Zonen zu definieren und Vermutungen
über die Dynamik der Keimzentren in den beiden untersuchten Organen
anzustellen.
Auf Grund der vorgelegten immunhistologischen Befunde lassen sich Follikel
grob in folgende Kategorien einteilen: In hyperplastischen Tonsillen kommen
Sekundärfollikel mit und ohne äußere Zone sowie zerfallende Follikel vor. Bei
Milzpräparaten treten Sekundärfollikel mit asymmetrischen oder symmetrisch
zurückgebildeten Keimzentren sowie Follikel mit sehr kleinen Keimzentrumsresten auf, die Primärfollikeln ähnlich sind, jedoch amyloidartiges Material enthalten.
Andere Arbeitsgruppen haben gezeigt, dass FDCs in Sekundärfollikeln von Tonsillen fünf verschiedene Zonen ausbilden können. Es handelt sich um die
dunkle Zone, die basale helle Zone, die apikale helle Zone, die äußere Zone und
die Korona oder Mantelzone. In der vorliegenden Arbeit lassen sich bei Tonsillenpräparaten in einem Follikel maximal vier Zonen gleichzeitig darstellen, da in den untersuchten Organen nicht alle Zonen im selben Follikel auftreten. In Milzpräparaten fehlt die durch CD23- FDCs charakterisierte äußere Zone; es lässt sich jedoch an der Follikeloberfläche eine innere Marginalzone mit CD21+ FDCs definieren. Dies entspricht der Auffassung anderer Autoren, von denen die äußere Zone als ein Tonsillen-spezifisches Kompartiment beschrieben wurde.
Die hier neu vorgelegten Resultate zeigen jedoch, dass die Mehrzahl der Follikel
in hyperplastischen Tonsillen keine äußere Zone besitzt. Es lässt sich daher
schließen, dass die äußere Zone ein stadienspezifisches und nicht ein
organspezifisches Kompartiment von Keimzentren darstellt. Darüber hinaus ist
es wahrscheinlich, dass die Keimzentren in hyperplastischen Tonsillen einen
Entwicklungszustand erreichen, der in der Milz normaler Erwachsener nicht
vorkommt.
Vermutlich unterscheidet sich die Dynamik von Sekundärfollikeln in Tonsillen
stark von derjenigen in anderen lymphatischen Organen. Dies wird durch den
Befund unterstützt, dass Tonsillen-Keimzentren durch fokale Apoptose von FDCs
und B-Lymphozyten im Verlauf einer Immunreaktion kollabieren und anscheinend akut zugrunde gehen. Die Keimzentren in den untersuchten Milzpräparaten von Erwachsenen erscheinen dagegen teilweise konzentrisch verkleinert, weisen jedoch keine apoptotischen Bereiche auf. Überreste komplett zerstörter Follikel finden sich in Milzpräparaten nicht. Die rückgebildeten Keimzentren der Milz erinnern immunhistologisch an Primärfollikel, enthalten jedoch teilweise ein amyloidartiges hyalines Material und eine ungewöhnliche Verteilung von CD23. Dies legt die Schlussfolgerung nahe, dass sich in weitgehend normalen Milzen erwachsener Patienten nur minimale B-Zellreaktionen abspielen und rückgebildete Sekundärfollikel unter Umständen langfristig persistieren.
Die durch FDCs vorgegebene Zoneneinteilung menschlicher Sekundärfollikel
hängt somit vom Stadium der jeweiligen B-Zellreaktion ab. Die Befunde zeigen
zum ersten Mal, dass Keimzentren bei starken akuten Immunreaktionen komplett
kollabieren anstatt sich langsam zurückzubilden. Untersuchungen während
massiver experimenteller Keimzentrumsreaktionen bei Nagetieren könnten zur
Aufklärung der zugrundeliegenden Mechanismen und ihrer therapeutischen Nutzbarkeit beitragen.
