Zusammenfassung:
Die AML M0 ist eine seltene Erkrankung mit schlechter Prognose. Sie stellt auf
klinischer und morphologischer Ebene eine heterogene Erkrankung dar. Die
Diagnose kann nicht allein aufgrund morphologischer Kriterien gestellt werden,
sondern bedarf zytochemischer und immunphänotypischer Zusatzuntersuchungen.
Bei der Tumorentstehung spielen neben Genmutationen
umschriebener Gene, z.B. durch Strukturaberrationen, auch komplexe
Veränderungen des Genoms mit Zugewinnen und Verlusten von Chromosomen
oder Chromosomenteilen eine wichtige Rolle. Der Nachweis einer Vielzahl von
klonalen Veränderungen reflektiert die biologische Heterogenität dieser
Krankheit und stellt einen weiteren Beweis dafür dar, das hier nicht ein
einheitlicher Subtyp vorliegt sondern ein ganzes Spektrum unterschiedlicher
maligner Prozesse in dieser Gruppe zusammengefaßt wird.
Chromosomale Veränderungen finden sich bei etwa der Hälfte der
diagnostizierten Patienten mit einer AML M0. Die konventionelle
Chromosomenanalyse ist nach wie vor die zentrale Methode der
Tumorzytogenetik, denn der Karyotyp stellt den wichtigsten prognostischen
Parameter, sowohl in bezug auf das Ansprechen der Therapie, als auch auf das
Überleben dar. Sie wird zunehmend durch moderne Methoden, wie die
Fluoreszenz-in-situ-hybridisierung oder Comparative-genom-hybridisierung
ergänzt. Ziel der Arbeit war herauszufinden, inwieweit die zytogenetischen
Veränderungen bei den von mir untersuchten Patienten mit einer AML M0 mit
denen in der Literatur übereinstimmen.
In den Jahren 1995-2000 wurden an die AG Tumorgenetik in Marburg 150
Proben aus ganz Deutschland mit einer bis dato nicht näher klassifizierten AML
eingesandt. Nach der zytogenetischen Untersuchung wurden die betreffenden
Kliniken um die Zusendung der kompletten Arztbriefe gebeten. Nach
Auswertung des vollständigen Krankenblattes konnten 17 Patienten dem AML
M0 Subtyp zugeordnet werden.
In der vorliegenden Arbeit wurden die Patienten anhand ihres zytogenetischen
Risikoprofils (gut-intermediär-ungünstig) in drei Gruppen eingeteilt.
Am häufigsten betroffen bei den von mir untersuchten Patienten war das
Chromosom 7. In der Literatur stellt die Monosomie 7 und die Deletion 7q die
häufigste Veränderung bei der AML M0 dar und wird anhand des Karyotyps der
Gruppe mit ungünstiger Prognose zugeordnet. Auch die Region q23-26 auf
Chromosom 11 war bei meinen Patienten regelmäßig betroffen. In der Literatur
sind diese Veränderungen oft mit Aberrationen von Chromosom 7 kombiniert,
was auch in der vorliegenden Arbeit der Fall war.
Komplex aberrante Karyotypen mit schlechter Prognose und fehlendem
Ansprechen auf die Chemotherapie zeigten sich bei vier Patienten. Eine
genaue Zuordnung zu einem FAB-Subtyp konnte in der Literatur nicht gefunden
werden. Bei den von mir untersuchten Patienten fand sich in drei Fällen ein
normaler weiblicher oder männlicher Karyotyp, die der Gruppe mit
intermediärem Risikoprofil zugeordnet wurden. Auch das ist bei einer AML M0
in 40-50% der Fälle zu finden. Neueste Forschungsergebnisse weisen aber auf
molekulargenetischer Ebene zunehmend Veränderungen in Form von
Fusionsgenen nach, die mit der klassischen Zytogenetik, selbst mittels CGH -
und FISH Technik, nicht nachweisbar sind. Hier gibt es bereits erste Versuche
auf molekulargenetischer Ebene in die Therapie einzugreifen. Es stellt sich
abschließend heraus, daß auch bei dieser kleinen untersuchten Gruppe es
typische Aberrationsmuster der einzelnen Chromosomen gibt und das
bestimmte chromosomale Regionen regelmäßig betroffen wurden, was mit der
Literatur übereinstimmt.
Für zukünftige Behandlungskonzepte der AML bleibt die Erforschung der
Leukämogenese und die Entwicklung neuer Behandlungsoptionen anhand der
Zytogenetik die wichtigste Aufgabe.
