Zusammenfassung:
Die Strategien in der Behandlung von Patienten mit M. Parkinson müssen bis heute als unbefriedigend betrachtet werden. Bei einer Vielzahl von Patienten kommt es im Verlauf der Behandlung zu Komplikationen. Ca. 30 % der Parkinson-Patienten entwickeln auf Grund der medikamentösen Therapie psychotische Symptome. Diese medikamentös induzierten Psychosen beeinträchtigen die Patienten oft sehr und reduzieren die Lebensqualität der Betroffenen enorm. Der Identifikation und bestmöglichen Behandlung dieser Symptome kommt daher ein hoher Stellenwert zu. Die Therapie sollte nach gängigen Empfehlungen darin bestehen, dass zunächst in vertretbarem Rahmen Anti-Parkinson-Medikamente reduziert werden und, wenn nötig, zusätzlich moderne atypische Neuroleptika verabreicht werden, nach derzeitigem Stand der Forschung vorzugsweise Clozapin oder Quetiapin. Mit dieser Arbeit wurde untersucht, wie Patienten mit M. Parkinson, insbesondere, wenn sie psychotische Symptome entwickeln, von ihren niedergelassenen Ärzten behandelt werden. Die Umsetzung erfolgte mittels eines eigens entwickelten Fragebogens, der an über 1000 Niedergelassene unterschiedlicher Fachgebiete versandt wurde. Es zeichneten sich hierbei deutlich Unterschiede hinsichtlich Diagnostik, Therapie und Verlauf zwischen den unterschiedlichen Fachgebieten und in Abhängigkeit von der Anzahl der behandelten Patienten ab. Es gelang mit der vorliegenden Arbeit zu zeigen, dass Ärzte, unter deren Patienten sich viele mit Morbus Parkinson befinden, sensibler auf die schwerwiegende Komplikation der Psychose reagieren, als Ärzte mit einem geringen Anteil an Parkinson-Patienten, obwohl gerade mit der Behandlung dieser schwerwiegenden Komplikation viel für die Patienten und ihre Lebensqualität erreicht werden könnte. Auch konnten Unterschiede zwischen den Ärzten in Abhängigkeit von deren Fachgebiet belegt werden. Es kann geschlussfolgert werden, dass ein Teil der Ärzte sich nicht genug mit dem sensiblen Thema „Psychose“ auseinandersetzten.
